THE PREVALENCE OF CHRONIC KIDNEY DISEASE AMONG PATIENTS WITH
CHRONIC HEART FAILURE

Introduction.
The relationship between the pathology of the cardiovascular system and the
kidneys is studied intensively (Kobalava Zh.D. et al. 2010). This led to the creation of cardiorenal
syndrome (Ronco C. et al, 2008). Cardiovascular disease is an independent risk factor for chronic
kidney disease (Elsayed E.F. et al, 2007).
Aim.
To study the prevalence of chronic kidney disease (CKD) among patients with chronic
heart failure (CHF).
Materials and methods.
We studied 60 patients (34 women and 26 men) with CHF of ischemic
origin (chronic coronary disease and acute myocardial infarction in history) with reduced, mid-range
and preserved ejection fraction (HFrEF, HFmrEF, HFpEF). Their age was 50-65 years (mean age 57.5
years). Patients with primary renal and renal vascular pathology were excluded. The examination of
patients was carried out on the basis of a therapeutic clinic. Patients underwent an echocardiography to
study the structural and functional parameters of the heart. The study of renal function included the
determination of serum creatinine concentration and glomerular filtration rate (GFR) according to the
EPI formula taking into account age, gender and race.
Results.
GFR was > 90 ml / min / 1.73 m2 in 20% of patients with CHF, 60-89 ml / min / 1.73
m2 in 40%, 45-59 ml / min / 1.73 m2 in 20%, 30-44 ml / min / 1.73 m2 in 6.7% and 15-29 ml / min /
1.73 m2 in 6.7%. Thus, CKD with GFR <60 ml / min / 1.73 m2 was observed in 33.4% of patients with
CHF. An analysis of the age-related features of the prevalence of CKD among patients with CHF
showed an increase in the frequency of GFR decline with an increase in the age of patients, as well as
with a decrease in the ejection fraction of the left ventricle (LVEF). Normal values of GFR in HFpEF
(LVEF> 50%) were observed in 60% of cases, with HFmrEF (LVEF - 40-49%) - in 20%, and with
HFrEF (LVEF <40%) - in none of the cases (p <0.05).
Conclusion.
CKD has a significant (33.4%) prevalence among patients with CHF. The age of
the patients and a decrease of LVEF are independent factors for the development of CKD in patients
with heart failure.
