Аннотация

Diabetes mellitus as a concomitant disease in COVID-19 is considered as one of the significant risk factors for adverse outcomes due to a more severe course of infection in the context of hyperglycemia and other aggravating factors in these patients, such as old age, obesity, high frequency of concomitant pathology (hypertension, cardiovascular diseases). There are currently virtually no verified data on the impact of different classes of sugar-lowering drugs on COVID-19 outcomes in DM. The fundamental issue of discontinuation or continuation of organoprotective therapy remains unresolved. In the human body, carnitine acts as a regulator of lipid metabolism, facilitating the transfer of long-chain fatty acids from the cell cytosol to the inner mitochondrial membrane, where they are involved in the P oxidation process and become a source for the synthesis of adenosine triphosphate - ATP. Levocamitine as a mitochondrial fatty acid transporter limits their availability for blood lipoprotein synthesis. Numerous clinical studies have examined the lipid-lowering effects of levocamitine in patients with kidney disease, diabetes mellitus, and hyperlipidemia.

Introduction.

Purpose of the study.

To evaluate the effectiveness of the use of energy-stimulating correction

of cardiovascular complications in type 1 diabetes mellitus in children with Covid-19.

Research materials and methods.

The paper presents an analysis of the results of observation,

laboratory methods of studies of 49 children with type 1 diabetes in children who have undergone Covid-
19, aged 4 to 18 years, the comparison group was 65 children with type 1 diabetes, the control group
was 30 practically healthy children of the same age.

Results of the study and their discussion.

According to the results of 4 weekly therapy in

patients with levocarnitine (2% solution at a dose of 200-300 mg 2 times a day) compared with the group
without levocarnitine, an increase in the concentration of anti-atherogenic HDL cholesterol was
observed (from 1,1±0,5 mmol/L to 1,29±0,5 versus 1,16±0,4 mmol/L, respectively).

After the course administration of levocarnitine, the level of triglycerides decreased slightly

(from 1,4±0,5 to 1,25±0,2 mmol/L against 1,3±0,4 mmol/L). The indicators of total cholesterol, LDL
cholesterol remained without significant changes. Metabolic disorders are the basis of the pathogenesis
of cardiovascular complications in children with DM. Levocarnitine replacement therapy adjusts energy
metabolism and is an effective method for preventing the development of DKAN in DM of Covid-19
survivors.

Conclusions

. Analyzing the data obtained, it can be concluded that metabolic changes are

indeed one of the pathogenetic mechanisms for the development of cardiovascular complications in
children with diabetes mellitus. Replacement therapy with the drug levocarnitine allows you to
adjust energy metabolism and is an effective way to prevent and reduce the development of
autonomous cardiovascular neuropathy in children with 1 DM type of Covid-19.
