LAPAROSCOPIC TREATMENT OF ADHESIVE DISEASE OF THE
ABDOMINAL CAVITY IN CHILDREN

Acute adhesive disease of the abdominal organs is one of the most severe
and common surgical childhood diseases, occurring at various times after surgery
on the abdominal organs, and continues to be an urgent problem in pediatric
surgery.
In the clinic of Fergana branch of the Republican Scientific Center for
Emergency Medical Care within 5 years, were treated 117 patients with adhesive
disease of the abdominal organs aged from 7 months up to 18 years old. There
were 44 girls, 73 boys. All children with adhesive disease of the abdominal organs
had previously undergone surgery for various diseases of the abdominal organs
(acute appendicitis, intestinal intussusception, peritonitis, injuries of the
abdominal organs). At admission, most patients had complaints characteristic of
intestinal obstruction: paroxysmal abdominal pain, repeated vomiting, impaired
stool and gas discharge. The diagnostic algorithm included a clinical study,
abdominal X-ray survey, ultrasound.
38 patients with adhesive intestinal obstruction underwent traditional
laparotomy. But the most effective was the use of laparoscopy in the elimination
of adhesive intestinal obstruction in 17 patients. It should be especially noted that
none of the patients underwent any decompression of the small intestine and
rather quickly was restored the passage through the gastrointestinal tract
(1-3 days). Minimal trauma to the anterior abdominal wall and abdominal organs
contributed to a smoother course of the postoperative period. The pain syndrome
was not expressed in all patients and was easily stopped by the introduction of
non-narcotic analgesics. Due to the smoother course of the postoperative period,
the length of stay of patients in the hospital was significantly reduced.
Thus, laparoscopic adhesiolysis is a safe, gentle and highly effective
treatment for acute adhesive intestinal obstruction in children in all age groups.
