Аннотация

The transformation and expansion of healthcare services across Uzbekistan—particularly in urban centers such as Tashkent—have led to a marked increase in the production of healthcare-related waste. This growth stems from the widespread adoption of disposable instruments, personal protective equipment, and single-use materials, contributing to both a shift in the morphological composition of healthcare waste and heightened public health risks. The complexity and infectious potential of this waste necessitate a robust, hygienic management system to prevent adverse environmental and epidemiological outcomes.

HEALTHCARE WASTE IN URBAN UZBEKISTAN: AN INTEGRATED

APPROACH TO RISK MITIGATION AND SUSTAINABILITY

Introduction:

The transformation and expansion of healthcare services across Uzbekistan—
particularly in urban centers such as Tashkent—have led to a marked increase
in the production of healthcare-related waste. This growth stems from the
widespread adoption of disposable instruments, personal protective equipment,
and single-use materials, contributing to both a shift in the morphological
composition of healthcare waste and heightened public health risks. The
complexity and infectious potential of this waste necessitate a robust, hygienic
management system to prevent adverse environmental and epidemiological
outcomes.

Of particular concern is the management of high-risk biological waste,

which carries pathogens capable of causing disease outbreaks if inadequately
contained. This thesis investigates the current state of healthcare waste
management in various types of public and private medical institutions in
Uzbekistan. The research also seeks to develop actionable recommendations for
aligning local practices with international hygiene and safety benchmarks,
focusing on sustainability, efficiency, and public health protection.

Main Part:

Overview of Healthcare Waste and International Standards

Healthcare waste, often containing infectious, toxic, or pharmaceutical

components, poses unique challenges in terms of sanitation, transportation, and
disposal. Globally, the Basel Convention serves as a foundational framework for
regulating hazardous waste, including waste from medical activities. Its call for
“Environmentally Sound Management” (ESM) practices emphasizes the need for
both containment and sustainability.

Uzbekistan has responded by implementing national guidelines,

particularly under SanPiN 0317-15, which categorizes medical waste into five
hazard classes and defines protocols for each stage of the waste cycle—from

segregation and temporary storage to disinfection and disposal. The system
mandates:

Hazard-based Segregation

using coded containers.

Sterilization

of Class B and C waste via physical or chemical treatment.

Secure Transport

procedures to prevent cross-contamination.

Routine Monitoring

by public health supervisory bodies.

Current Challenges in Waste Management

Despite the existence of these regulations, widespread discrepancies

remain in their execution. Issues include:

Weak enforcement of segregation policies.

Inconsistent access to disinfection technologies in rural or underfunded

regions.

A lack of integrated waste treatment systems linking smaller facilities to

centralized processing hubs.

Field data gathered from healthcare facilities revealed disparities in waste

generation rates:

Public Institutions:

o

Multispecialty hospitals: 0.35 ± 0.21 kg/bed-day

o

Infectious disease centers: 1.01 ± 0.54 kg/bed-day

o

Maternity hospitals: 1.52 ± 0.31 kg/bed-day

Private Institutions:

o

Outpatient clinics: 0.30 ± 0.18 kg/bed-day

o

Specialized centers: 0.85 ± 0.50 kg/visit

A modified calculation method was introduced that accounts for patient

turnover and the intensity of service provision. This formula enables more
accurate risk forecasting and supports the creation of localized sanitary control
strategies.

Innovative Solutions and Recommendations

To address these gaps, this study proposes a hybrid waste management

system, wherein:

Up to 75% of medical waste is disinfected directly on-site using compact

treatment units.

The remaining 25%—classified as highly infectious—is transferred to

centralized facilities with specialized processing capabilities.

Additionally, the study underscores the importance of:

Developing institutional training programs on hygienic waste handling.

Enhancing interdepartmental cooperation to coordinate waste logistics.

Implementing community education campaigns to increase awareness

among staff and local populations.

Results and Conclusion:

The findings emphasize the urgent need to restructure current medical

waste handling protocols through a balanced combination of decentralization
and centralized oversight. The study's framework was presented at the 2024
KOR-UZB Conference on Environmental Safety and was acknowledged for its
technical innovation during the Techno Ways Marathon on Sustainable
Healthcare Solutions.

Adopting an integrated system for healthcare waste management—

grounded in evidence-based hygiene standards and sustainable practices—can
significantly reduce health risks and environmental contamination. Moreover,
investing in human resource development will foster long-term institutional
resilience and compliance with evolving international guidelines.

Future research should evaluate the long-term efficacy of proposed

systems, explore automation in waste processing, and test eco-friendly
treatment alternatives such as plasma incineration or enzymatic digestion
technologies.
