EFFECTIVENESS OF USING VARIOUS METHODS OF ADAPTATION OF
CHILDREN TO DENTAL RECEPTION
Key words:
distribution, intensity of caries, maxillodental abnormality in children, effectiveness
of planned stomatologic sanation, organized children’s collectives.
Introduction.
Pediatric dentistry is one of the most complex modern dental areas due to
the fact that the success and quality of treatment is completely determined by the patient's
willingness to cooperate with the dentist. As practice shows, the lack of preliminary adaptation
of children to a dental appointment can cause poor-quality or incomplete treatment, which leads
to both premature loss of temporary teeth and the development of a persistent phenomenon of
dentophobia. Therefore, today, pediatric dentistry is in a continuous search for multiple effective
methods of adaptation, since each child requires an individual approach, but his treatment a
priori should take place in comfortable conditions accessible assistance in the treatment of dental
diseases was provided, children were monitored dynamically, especially for dispensary groups,
the dentist carried out health education and preventive work among children, their parents and
teachers in health lessons. With the introduction of market relations, the SHSK began to close at
the initiative of school directors, they began to rent them out to private dentists, some offices
were re-equipped. The dental service lost its "branches" in schools, and children stopped
receiving dental care, since parents often could not take time off from work and take their child
to the dental clinic not only for preventive measures, but also for treatment. The child began to
visit the dental clinic only in critical cases with acute toothache.
To identify the effectiveness of adaptation to dental treatment of children aged 5-6 years using
various techniques.
Materials and methods.
In 2023, 46 patients aged 5 to 6 years were treated at the
Children LLC clinic in Tashkent. Three study groups of children were identified depending on
the choice of adaptation technique: 1) ZAKS (sedation with a mixture of nitric oxide and oxygen)
- 17 patients; 2) preliminary familiarization of children with a dental appointment based on the
"tell-show" principle - 18 patients; 3) treatment in conditions of no adaptation - 13 patients. The
diagnoses of the teeth to be treated according to the ICD-10 classification were dentin caries
(K02.1) and pulpitis (K04.0). The effectiveness of the methods was assessed during two dental
appointments prescribed for each of the study patients with an interval of 1-2 weeks. The
average duration of a visit to the dentist was 30-45 minutes; during each visit, 1 tooth was treated
using local infiltration anesthesia with a 6% articaine solution (Articaine INIBSA).

Results.
The following data were obtained during the study. In Group 1, it was possible
to perform full dental treatment in 14 patients (74%), in Group 2, in 9 patients (62%), and in
Group 3, in 5 patients (33%).
Conclusions.
Summarizing all of the above, taking into account the growth in the
prevalence and intensity of caries, as well as the assessment of the state of dental care in
organized children's groups, we can confidently say about the need for preventive work of a
dentist (dentist) in school dental offices to identify dental diseases, their treatment and
dispensary observation of sick children, which subsequently reduces the costs of the state for
prosthetics of adult patients
.
Thus, the most effective method of adapting children to a dental
appointment in this study was sedation with a mixture of nitric oxide and oxygen. In the absence
of ZAKS in dental practice, children can also be successfully adapted using the "tell-and-show"
technique. Its effectiveness lies in the fact that the doctor shows the child the instruments used in
the treatment, the principle of operation, and allows them to try them on phantoms of the
dentition in order to eliminate in the future the fear of the unknown before the upcoming
manipulations. This technique can be combined with the use of ZAKS, which increases the
effectiveness of the dental manipulation. In the absence of adaptation of children to a dental
appointment, the study obtained the lowest percentage of successful treatment. Most of the
studied patients refused to cooperate; dental manipulations were interrupted in 4 patients.
Therefore, in addition to manual skills, a modern pediatric dentist must have effective adaptation
techniques, which often play a key role in the successful treatment of children of different age
groups.
