VAGINAL MICROFLORA IN BACKGROUND DISEASES OF THE
CERVIX

As you know, the main direction in the prevention of cervical cancer is the active detection of
patients with background and precancerous diseases, especially women who have long-term changes in
the cervix against the background of chronic infections of the lower genital tract.
The purpose of the study:
to assess the nature of changes in the vaginal microflora in background
diseases of the cervix.
Materials and methods:
we examined 58 women with cervical ectopia aged 25 to 45 years. The
identification of the Amsel criteria for the diagnosis of bacterial vaginosis, simple and extended
colposcopy, microscopy of vaginal smears by Gram, and cytological evaluation of cervical smears were
performed.
The results of the study and their discussion.
As a result of complex clinical and diagnostic
studies, 26 (70.8%) women were found to have certain diseases, including:
exocervicitis in 6 (27%), endometriosis of the cervix in 4 (5.5%), polyp of the cervix in 2 (3.2%), ectopia
in 21 (33.4%), old ruptures and scars of the cervix in 3 (4.8%), narrowing and overgrowth of the cervix
in 2 (3.2%), papilloma of the cervix in 1 (2.4%), cervicitis caused by herpetic and fungal infection in 12
(18.3%), leukoplakia in 1 (2.4%) women.
The pH value up to 4.5 was found in 15% of women, in the range of 4.5-6 in 68.3%, pH>6 in 16.7%
of the examined women.
White blood cells in the smears were single (38.3%), less than 10 in the visual field (53.3%) and
10-20 (8.4%), epithelial cells in moderate (36.7%) and large (63.3%) amounts. "Key" cells characteristic
of bacterial vaginosis (epithelial cells covered with a large number of adherent gramvariable
microorganisms) were found in 81.7% of smears. The qualitative assessment of the microflora revealed
the predominance of gram-positive bacilli (the morphotype of lactobacilli) in only 15% of the smears,
while gram-negative bacilli dominated in 64.7%, and representatives of the coccal microflora in 35.3%
of the smears. Yeast-like fungi of the genus Candida were found in 15.7% of smears in the form of
budding forms - the intermediate phase of vegetation.
Conclusion.
Thus, in women with impaired vaginal microbiocenosis, we found more signs that
cause suspicion of cervical cancer: dysplastic tissue changes and an increase in the number of abnormal
colposcopic results.
