Аннотация

Endokrin oftalmopatiya (EOP) qalqonsimon bezning disfunksiyasi natijasida kelib rivojlanuvchi autoimmun kasallik bo'lib, surunkali progressiv shaklda orbita yumshoq to‘qimalarining yallig‘lanishi va ekstraokulyar mushaklar (EOM) qalinlashishi bilan namoyon bo‘ladi. Oxirgi o‘n yillik davomida EOP asosan yosh, mehnatga layoqatli va reproduktiv yoshdagi odamlar o‘rtasida ko‘paymoqda [1]. Ilmiy adabiyotlarda EOP fonida ko‘z olmasi hamda uning yordamchi qismlarida kuzatiladigan patologik jarayonlar haqida ko‘plab ma’lumotlar keltirilgan, lekin ular o‘rtasidagi bog‘liqlik (masalan: ko‘z ichki bosimi, EOP og‘irlik darajasi va EOM shishi) haqidagi tadqiqot natijalari kamchilikni tashkil qiladi [2,3,4,5].  Klinik tadqiqotlar xulosasiga ko‘ra EOP kasalligi bor bemorlarning ¼ qismida ko‘z ichki bosimi oshishi aniqlangan. Buning asosiy sababi sifatida esa quyidagilar ko‘rsatib o‘tilgan: orbital venoz dimlanish, orbital yumshoq to‘qimalar hajmining oshishi va shishgan EOMning ko‘z olmasini bosishi [6,7,8].

Annotatsiya.

Endokrin oftalmopatiya (EOP) qalqonsimon bezning

disfunksiyasi natijasida kelib rivojlanuvchi autoimmun kasallik bo'lib, surunkali
progressiv shaklda orbita yumshoq to‘qimalarining yallig‘lanishi va
ekstraokulyar mushaklar (EOM) qalinlashishi bilan namoyon bo‘ladi. Oxirgi o‘n

kamchilikni tashkil qiladi [2,3,4,5]. Klinik tadqiqotlar xulosasiga ko‘ra EOP
kasalligi bor bemorlarning ¼ qismida ko‘z ichki bosimi oshishi aniqlangan.
Buning asosiy sababi sifatida esa quyidagilar ko‘rsatib o‘tilgan: orbital venoz
dimlanish, orbital yumshoq to‘qimalar hajmining oshishi va shishgan EOMning
ko‘z olmasini bosishi [6,7,8].

tireotoksikoz fonida endokrin oftalmopatiya (EOP) bilan kasallangan 72 (144 ta
ko‘z) nafar bemorlarda o‘tkazildi. Klinik tadqiqotda 20 yoshdan 71 yoshgacha
bo‘lgan 54 nafar ayol va 18 nafar erkak qatnashdi. Bemorlarning o‘rtacha yoshi
44,8 ± 1,4 yoshni tashkil qildi.

Barcha bemorlar ko‘zining holati umumiy

oftalmologik tekshiruvlar orqali baholangan. Shuningdek, bemorlarda
ekstraokulyar mushaklarning biometriyasi skleradan 6-8 mm uzoqlikda
Accutome B-ScanPlus (AQSh) apparati yordamida 10 dan 65 dB gacha
kuchlanish diapozonidan foydalanildi.

Bemorlarga tashxis qoʻyishda xalqaro

NOSPECS (S.Werner, 1977) tasnifidan foydalanildi.

Tadqiqot natijasi.

NOSPECS tasnifi bo‘yicha bemorlarda quyidagi

patologik holatlar aniqlandi: 15 ta bemorda yuqori qovoq retraksiyasi, 10 ta
bemorda xemoz va konyunktiva ineksiyasi, 46 ta bemorda ekzoftalm, 8 ta
bemorda keratit va 6 ta bemorda esa ko‘rish o‘tkirligining pasayishi.

Tadqiqot davomida 26 nafar (36%) bemorlarda EOP fonidagi ikkilamchi

glaukoma aniqlandi. Shundan 14 nafar bemor I bosqich, 12 nafarida esa II
bosqich glaukoma tashxisi tasdiqlandi. Lekin glaukomaning III va IV bosqichi
tadqiqot davomida aniqlanmadi. Norma darajadagi ko‘z ichki bosimi 56 nafar
bemorda (n=78%) aniqlandi (18,4±4,55 mm sim.ust). Ko‘z ichki bosimini
oshishi (26,0 mm sim.ust.dan ortiq) 14 ta (n=22%) bemorda aniqlandi va ularga
antigipertenziv terapiya tavsiya etildi

Shuningdek, bemorlarning endokrin oftalmopatiya darajasi va

ekstraokulyar mushaklar exografik tekshiruvi va KIB natijalari o‘rtasida to‘g‘ri
kuchli bog‘liqlik (P=0.001,R=0,38 ) aniqlandi.

Xulosa.

Endokrin oftalmopatiya tashxisli bemorlarda ko‘z olmasi va orbita

yumshoq to‘qimalarining ultratovush diagnostikasi, ko‘z ichki bosimi
monitoringi va kasallikning klinik kechishini bir vaqtda baholash orqali
ikkilamchi glaukoma rivojlanish xavfini aniqlash mumkin. Bu esa EOP fonida
rivojlanuvchi ikkilamchi glaukomani diagnostikasi (perimetriya, oftalmoskopiya
va tonometriya) uchun qo‘shimcha mezon sifatida xizmat qiladi va EOPning
asoratlarini davolash uchun klinik amaliyotda keng foydalanishga tavsiya etilishi
mumkin.
