Аннотация

Endometrial retseptivlik – bu bachadon shilliq qavatining embrionni qabul qilish va muvaffaqiyatli implantatsiya qilishga tayyor holatidir. Ushbu holatning buzilishi bepushtlikning asosiy sabablaridan biri hisoblanadi, ayniqsa, endometrioz, surunkali endometrit yoki yupqa endometriy fonida kuzatiladi. Retseptivlik buzilganda, hatto sog‘lom va sifatli embrion ham endometriyga yopishmaydi.

Kirish:

Endometrial retseptivlik – bu bachadon shilliq qavatining embrionni qabul qilish

va muvaffaqiyatli implantatsiya qilishga tayyor holatidir. Ushbu holatning buzilishi
bepushtlikning asosiy sabablaridan biri hisoblanadi, ayniqsa, endometrioz, surunkali
endometrit yoki yupqa endometriy fonida kuzatiladi. Retseptivlik buzilganda, hatto sog‘lom va
sifatli embrion ham endometriyga yopishmaydi.

Trombotsitlarga boy plazma (TBP) terapiyasi — bu regenerativ va immunologik ta’sir

ko‘rsatish orqali endometriy funksiyasini tiklashda istiqbolli biotexnologik usuldir. TBP
tarkibidagi o‘sish omillari va sitokinlar endometriyni yangilaydi, immun muhitni
muvozanatlashtiradi hamda ildiz hujayralarni faollashtiradi.

: Endometrial retseptivlik buzilgan bemorlarda TBP terapiyasining

samaradorligini oshirish uchun regenerativ va immun yondashuvlarni baholash hamda klinik
qo‘llash protokollarini takomillashtirish

.

Material va metodlar

: 2018–2024 yillarda chop etilgan ilmiy maqolalar asosida tizimli

adabiyotlar sharhi o‘tkazildi. Ma’lumotlar PubMed, ACOG va Dissercat.com bazalaridan olindi.
Asosiy e’tibor TBPning endometriy retseptivligi, immun javob va implantatsiya
samaradorligiga ta’siriga qaratildi.

Natijalar

: Tahlil qilingan ilmiy manbalar va eksperimental ma’lumotlar asosida shuni

ta’kidlash mumkinki, endometrial retseptivlik buzilgan bemorlarda trombotsitlarga boy
plazma (TBP) terapiyasi bir nechta darajadagi regenerativ va immunologik mexanizmlar orqali
klinik samaradorlikni oshiradi.

1. Regenerativ ta’siri: TBP tarkibida mavjud bo‘lgan o‘sish omillari (VEGF, TGF-β1, PDGF,

FGF, CTGF) hujayra proliferatsiyasini kuchaytiradi va endometriy to‘qimalarining
yangilanishini faollashtiradi. Kollagen sintezi TGF-β1 va Smad2/3 signal yo‘llari orqali
faollashadi, bu esa endometriy strukturasini tiklashda muhim rol o‘ynaydi.Angiogenez TBP
tomonidan kuchli qo‘llab-quvvatlanadi. Yangi qon tomirlar hosil bo‘lishi to‘qimalarga kislorod
va oziqa yetkazilishini yaxshilaydi, bu esa implantatsiya uchun qulay muhit yaratadi.TBP
terapiyasi yupqa endometriy muammosida endometriy qalinligini ko‘rsatkichlar bo‘yicha 6–9
mm gacha yetkazishga yordam berganligi klinik sinovlar bilan tasdiqlangan.

2. Immunomodulyatsiya: Endometriy retseptivlik davrida immun tizim muvozanatli

faoliyat yuritishi kerak. TBP bu holatga ta’sir etib, uNK (uterine natural killer) hujayralari va T-

regulyator (Treg) hujayralari sonini ko‘paytiradi.uNK hujayralar angiogenezni rag‘batlantiradi,
Treg hujayralar esa embrionni begona antigen sifatida rad qilinishining oldini oladi.TBP
tarkibidagi IL-10, TGF-β kabi antiinflamator sitokinlar orqali surunkali yallig‘lanish holatlarini
bartaraf etish mumkin, bu ayniqsa surunkali endometrit fonida muhim ahamiyatga ega.

3. Klinik natijalar:TBP terapiyasidan keyin endometriy retseptivligining yaxshilanishi

kuzatilgan va bu embrion implantatsiyasi muvaffaqiyati 30–50% gacha oshganini
ko‘rsatmoqda (meta-tahlillar asosida).Repetitiv implantatsiya muvaffaqiyatsizligi (RIF) bo‘lgan
ayollarda TBP yuborilgach, homiladorlikka erishish darajasi sezilarli oshgan. Ba’zi holatlarda
TBPni 2–3 marta yuborish optimal natija bergan.TBP yuborishning faollashtirilgan shakli
(CaCl₂ bilan) oddiy PRPga qaraganda yuqori biologik faollikka ega bo‘lib, hujayra darajasidagi
tiklanish jarayonlarini tezlashtiradi.

4. Individuallashtirilgan yondashuv: PRP terapiyasining maksimal foydali bo‘lishi uchun

bemorning endometriy qalinligi, yoshi, surunkali yallig‘lanish mavjudligi, avvalgi IVF natijalari
va immun profili inobatga olinishi lozim.

Dozalash, yuborish soni va protokollarning shaxsga mos tanlanishi —

personalizatsiyalashgan reproduktiv tibbiyot doirasida muhim bosqich hisoblanadi.

Xulosa:

TBP terapiyasi orqali retseptivlikni tiklash, immun muvozanatni ta’minlash va

implantatsiyani

qo‘llab-quvvatlash

mumkin.

Faollashtirilgan

TBP

yordamida

regeneratsiyalovchi omillar, angiogenez va immunologik himoya mexanizmlari birgalikda
harakat qilib, bepushtlikni davolashda yangi klinik imkoniyatlar ochadi.
