Аннотация

The urgency of the problem of chronic urticaria in children is due to the heterogeneity of etiological factors, age-related features of the immunopathological mechanisms of development and course [4,8,9,11,13,14,18]. According to clinical recommendations: urticaria in children [3] and international recommendations [1], urticaria that persists or recurs for more than 6 weeks is considered chronic urticaria.  Among children with recurrent urticaria, atopy is detected in 33% of patients.

The urgency of the problem of chronic urticaria in children is due to the

[4,8,9,11,13,14,18]. According to clinical recommendations: urticaria in children
[3] and international recommendations [1], urticaria that persists or recurs for
more than 6 weeks is considered chronic urticaria. Among children with
recurrent urticaria, atopy is detected in 33% of patients. Chronic forms of
urticaria in children under 2 years of age are rarely diagnosed; In children over
2 years of age, acute forms dominate over chronic ones, and in adolescents and
children over 12 years of age, the prevalence of chronic forms of urticaria
prevails [2,5,6,7,9,10,12,15,16,17]. A characteristic feature of recurrent atopic
urticaria is a hereditary burden of atopic diseases, an increased level of total
immunoglobulin E in the blood, a decrease in the number of cells belonging to
the T-suppressor subpopulation of lymphocytes, positive skin tests with various
allergens of different groups — food, household, pollen, epidermal.

The

aim

is to study the state of allergic sensitization in children with

chronic recurrent urticaria (CRU) depending on H.pylori.

Materials and methods of research

. The study included 63 children with

CRU who were hospitalized in the pediatric allergy department and 30
practically healthy children over the age of 12. Of the children with IBS, 46 were
with confirmed H.pylori (group I), and the remaining 30 were patients without
H.pylori (group II). The presence of sensitization and autoimmune process was
revealed by the activity of specific immunity indicators – the number of antigen-
binding lymphocytes (ASL) by indirect rosette formation to H.pylori according to
the method of Gharib F. Yu. et al., 1983.

Results and discussions.

The presence of eosinophilia in the hemogram

of group I children with an index of 7.14±0.2% was found only in 17.2±3.9% of
group I children (versus 9.1±5.1%), p<0.05. It was revealed that in children with
CRC, the content of ASL to H.pylori in children of group I was significantly higher

than in patients of group II and practically healthy children (p<0.001). It should
be noted that in children of group I, the content of ASL among all the studied
antigens was the highest for H.pylori. The content of ASL to H.pylori, depending
on the degree of disease activity in group I children, was significantly higher
(p<0.001) than in practically healthy children. Moreover, the highest values of
ASL for H.pylori were found in children with severe CRC activity, which was
28.6±1.5%. The parallelism we found between the degree of disease activity and
the content of ASL to the studied antigens indicates the presence of sensitization
and autoimmune reactions in association with H.pylori in children with CRU.

Conclusion.

In children with CRU, direct correlations of H.pylori with the

prolongation of both the pathological process in the intestine and in the whole
div have been identified. These data clearly emphasize the importance of
timely elimination of H.pylori in intestinal diseases. At the same time, the
appointment of a specific drug (regimen) should take into account its
bioavailability and effectiveness.
