Research Objective

: To investigate the clinical characteristics, seizure types, etiological

factors, and treatment responses in children with epilepsy, with a focus on optimizing
individualized therapeutic strategies.

Introduction

: Epilepsy is one of the most common chronic neurological disorders in the

pediatric population, affecting approximately 0.5–1% of children worldwide. It encompasses a
diverse group of syndromes characterized by recurrent, unprovoked seizures resulting from
abnormal electrical activity in the brain. The burden of epilepsy extends beyond seizures, affecting
cognitive, behavioral, and psychosocial development.

Pediatric epilepsy poses unique diagnostic and therapeutic challenges due to

developmental variations in seizure expression, evolving semiology, age-specific etiologies, and
potential neurodevelopmental implications. Early diagnosis, precise classification, and tailored
treatment plans are essential for achieving optimal seizure control and minimizing long-term
sequelae.

This thesis aims to provide a comprehensive overview of pediatric epilepsy based on

clinical evaluation and therapeutic outcomes in a regional cohort, thereby contributing to improved
clinical management and quality of life for affected children.

Materials and Methods

: This prospective observational study was conducted at the

Pediatric Neurology Clinic of Samarkand State Medical University over a 30-month period.

Study Population: A total of 94 children aged between 1 and 16 years diagnosed with

epilepsy were included. The diagnosis was based on the International League Against Epilepsy
(ILAE) 2017 classification criteria.

Inclusion Criteria

:

1.

Children aged 1–16 years with ≥2 unprovoked seizures >24 hours apart

2.

Normal metabolic and structural brain evaluation (where applicable)

3.

Informed parental consent

Exclusion Criteria

:

a.

Acute symptomatic seizures (e.g., febrile seizures)

b.

Epilepsy secondary to progressive neurodegenerative disorders

c.

Noncompliance with follow-up

Assessment Tools and Procedures

:

1. Detailed clinical history and seizure semiology
2. Neurological examination
3. Electroencephalogram (EEG) recordings (routine and sleep-deprived)

4. Neuroimaging (MRI) where indicated
5. Etiological workup including metabolic, infectious, and genetic testing
6. Classification of seizure type and epilepsy syndrome (ILAE 2017)
7. Treatment regimens and response monitoring over 12 months

Results

: Of the 94 children, 51 (54.3%) were boys and 43 (45.7%) were girls. The mean

age at seizure onset was 5.2 ± 3.1 years. Generalized seizures were observed in 62% of patients,
with tonic-clonic seizures being the most prevalent type. Focal seizures were recorded in 28%, and
mixed types in 10%.

Etiological Distribution

:

a.

Idiopathic epilepsy: 48%

b.

Structural (congenital malformations, perinatal hypoxic injury): 29%

c.

Genetic: 12%

d.

Metabolic/infectious: 11%

Electroencephalographic Findings

:

a.

Interictal epileptiform discharges (IEDs) were observed in 79% of patients

b.

Generalized spike-wave complexes: 58%

c.

Focal spikes: 28%

d.

Multifocal abnormalities: 13%

Imaging Results

: MRI abnormalities were found in 31% of cases, mainly cortical

dysplasia, hippocampal sclerosis, and periventricular leukomalacia.

Treatment and Outcome: All patients were started on antiepileptic drug (AED)

monotherapy or polytherapy based on seizure type and etiology. The most commonly used AEDs
were valproate, levetiracetam, carbamazepine, and oxcarbazepine.

Monotherapy success (seizure-free >12 months): 58%

b.

Polytherapy required: 42%

c.

Drug-resistant epilepsy: 14% (criteria: failure of ≥2 appropriate AEDs)

Adverse Effects and Tolerability

:

a.

Sedation: 17%

b.

Weight gain: 12%

c.

Behavioral disturbances (agitation, irritability): 9%

d.

No severe idiosyncratic reactions observed

Conclusion

: Pediatric epilepsy exhibits diverse clinical and etiological profiles,

demanding individualized diagnostic and treatment strategies. The predominance of generalized
tonic-clonic seizures and idiopathic etiologies reflects typical trends in regional populations.

EEG remains a cornerstone of diagnosis, with characteristic epileptiform discharges

guiding both syndrome classification and treatment choice. Structural imaging further aids in
identifying causative lesions in focal epilepsy cases, influencing both medical and surgical
management options.

Monotherapy remains effective for seizure control in over half of pediatric patients,

reinforcing the principle of starting treatment with the most appropriate single agent. However, a
substantial subset requires combination therapy due to multifocal or refractory epilepsy patterns.

Clinical vigilance is essential in early detection of drug-resistant epilepsy, prompting

timely referral for advanced diagnostics and consideration of non-pharmacological therapies (e.g.,
ketogenic diet, vagus nerve stimulation, epilepsy surgery).

The findings underscore the necessity of a multidisciplinary approach involving

neurologists, pediatricians, psychologists, and educational specialists to support holistic care.

Regular follow-up, parental education, and psychosocial support significantly enhance

long-term outcomes.

Ultimately, early diagnosis, seizure-type specific therapy, and personalized management

plans remain the key to improving prognosis and quality of life for children living with epilepsy.
