POLIMIKROGIRIYALI FARMAKOREZISTENT EPILEPSIYANI DAVOLASH NATIJALARI

Polimikrogiriya (PMG) korteksning (MC) malformatsiyasi bo'lib, g'ayritabiiy postmigratsiya natijasida ikkinchi darajali MCI sifatida tasniflanadi. neyron rivojlanishi [[1], [2], [3], [4]]. Nevrologik kasalliklar spektriga qo'shimcha ravishda, PMG bilan og'rigan bemorlarning 65-87% epilepsiyani rivojlantiradi va ko'pchilik hayotning birinchi besh yilida tutqanoqni boshdan kechiradi [1]. PMG bilan bog'liq epilepsiya bilan og'rigan bemorlarning taxminan 65 foizida dori-darmonlarga chidamli epilepsiya (DRE) mavjud. EMH bilan bog'liq PE [1,5,6] bo'lgan bemorlarda epilepsiyani jarrohlik yo'li bilan davolashni ko'rib chiqish kerak. MCI bilan bog'liq ED [[7], [8], [9]] bo'lgan to'g'ri tanlangan bemorlarning kichik guruhida epilepsiya jarrohligi doimiy antikonvulsant davolashdan ustun ekanligi ko'rsatilgan bo'lsa-da, MCI bilan bog'liq epilepsiya operatsiyalarining atigi 5 foizi MCI bilan bog'liq epilepsiyaga qaratilgan [10]. Ushbu bemorlarda jarrohlikning murakkabligi ko'p hollarda keng tarqalgan (ba'zan ikki tomonlama) miya malformatsiyasining mavjudligini va PMH korteksining odatda normal miya funktsiyasini saqlab turishini aks ettiradi [[1], [11]]. Ushbu xavotirlarni hisobga olgan holda, tibbiy yordam ko'rsatuvchi provayderlar ERF bo'lgan bemorlarning ushbu guruhida jarrohlik amaliyotini o'tkazishni istamasligi mumkin va shuning uchun jarrohlikdan kam foydalaniladi yoki kechiktiriladi [10].
