FEATURES OF COPD STRUCTURE IN ELDERLY PATIENTS

Abstract:
Currently, there is a clear trend towards
an increase in the elderly population. Their share
in the population of many countries has exceeded
20%. Prevention of aging is one of the most
actively and dynamically developing new areas of
medicine in recent years. Factors such as
decreased metabolism, dysfunction of the
endocrine system and immunity, impaired
microcirculation and neurohumoral regulation
play a role in the process of natural aging. The age-
related decrease in estrogen levels in a woman's
div plays an important role in the development
of endogenous changes in COPD. Skin aging plays
an important role in the mechanism of the overall
biological aging process of the div. On the one
hand, the mucous membrane of the oral cavity
determines metabolic, structural and functional
changes common to the entire div, and the
condition of the skin as a whole may indicate the
degree of aging of the div. On the other hand, the
oral cavity is an organ that is constantly in contact
with the external environment, as a result of
which signs of aging can appear much earlier than
in other organs.
INTRODUCTION
The oral cavity is sensitive to estrogen and reactive organs due to its specialized receptors. Estrogen
stimulates the proliferation of epithelial cells, pigmentation of the skin, has a positive effect on its
hydration and turgor, promotes angiogenesis and increases the level of hyaluronic acid in the dermis.

A decrease in estrogen levels is one of the important causes of skin aging.1. Hormone-dependent aging
occurs independently of photoaging and is expressed in thinning and deterioration of the skin.
The purpose of the study. To conduct a comparative analysis of data from morphological studies of
COPD in patients with normal and reproductive dysfunction.
Materials and methods of research.
The aim of the study was biopsies of the oral mucosa obtained
from 21 women aged 68 to 21 years. When selecting patients for this study, criteria such as age, the
state of the reproductive system, and somatic diseases were taken into account.
Histological examination of biopsies was performed to identify morphological changes in epithelial
cells. To detect changes in the thickness of the epidermis, the sections were stained with hematoxylin
and eosin. To identify and determine the degree of degeneration of collagen and elastic fibers of the
dermis, sections were stained with hematoxylin, picrofoxin and orcaine. Microscopic examination was
performed using gastroscopy.
The results of the study and their discussion. The features of the COPD structure in patients were
compared depending on the state of the reproductive system. At the same time, a comparative analysis
was carried out in patients with a normal menstrual cycle and in patients with age-related changes in
the menstrual cycle (before and after menopause). The boundary between the epithelium and the
papillary layer of the SOPR was unchanged in 83.8% and smoothed in 16.2% of the examined patients.
In 62.2% of patients with a normal menstrual cycle, the structure of collagen fibers of the dermis
corresponds to the norm, and a weak degree of its dystrophy was detected in 30.6% of the examined,
an average degree in 7.2%. In 46.2% of representatives of this group, the elastin fibers in the
preparation did not change. In the same percentage of the examined women, the signs of elastosis were
weakly pronounced and in 7.6% they were moderately pronounced. When. The morphological features
of COPD in patients with age-related reproductive dysfunction were analyzed, and the following results
were obtained. Received. The morphological structure of collagen fibers corresponded to the norm in
76.2% of the examined patients, and in the remaining 23.8% of patients, a weak degree of degeneration
of collagen fibers was revealed. Normal elastin fibers have been detected. In 59.2% of cases, mild
elastosis was detected in 5.9% of patients, and moderate severity in 34.7% of cases. A comparative
analysis of the data from morphological studies of COPD in patients with normal and reproductive
dysfunction revealed the following functional patterns: collagen fibers of the oral mucosa did not
change in 1% of patients in the 61.5 group and in 2% of patients in the 76.5 group. There was no
statistically significant difference between the groups in the degree of degeneration of collagen fibers

(p=0.526). The elastin fibers of the mucous membrane corresponded to the norm in 46.2% of patients
with preserved reproductive function and in 58.8% of patients before and after menopause. A mild
degree of elastosis was detected in representatives of group 1 almost 8 times more often than in
representatives of group 2, and vice versa, moderate elastosis was detected in representatives of group
2 almost 4.5 times more often than in patients of group 1. In general, the difference in the degree of
elastosis in patients of the two compared groups was statistically significant (p=0.027).
CONCLUSION
Thus, a comparison of the structural features of SOPR biopsies in patients revealed statistically
significant differences between the groups in terms of the degree of elastosis of the dermis of the skin,
depending on the state of the reproductive system. As for the changes in the thickness of the epidermis,
certain differences between the groups of examined patients attracted attention. A comparison of skin
biopsies in patients with a normal menstrual cycle and in patients with age-related changes in the
menstrual cycle revealed a statistically significant difference in the degree of skin elastosis between the
groups (p=0.027). It is worth noting that the thickness and/or number of elastin fibers in women with
disabilities are clearly increased. Reproductive function was detected almost 4.5 times more often than
in patients with a normal menstrual cycle. Confirmation of this fact is given in the literature, which
shows that a decrease in estrogen levels is one of the important causes of aging in COPD 1.The latter
hormone-dependent aging manifests itself in the thinning of the mucous membrane and the destruction
of elastin and collagen fibers.
