  • Among the injuries of the facial skeleton, fractures of the mandible are most common, accounting for 70% to 85% according to the data of domestic and foreign clinics.   • Recent studies using the dPTFE( dense polytetrafluoroethylene ) membrane have shown a positive result at the site of regeneration even if the membrane itself is exposed in repaired period. With this type of complication the membrane is removed at a period of 4-6 weeks and replaced with a collagen membrane with subsequent approximation of the edges of the flap by sutures [8]. This article shows the possibility of regenerating the bone graft from sole xenogenic material using dPTFE membrane in case wound dehiscence in repaired period but without using the collagen membrane coating step after removing dPTFE and it healing by secondary tension.

  • The purpose of our study was to study the peculiarities of the biophysical properties and the mineralizing potential of the oral fluid in children during the period of destructing permanent teeth. Mixed saliva performs a mineralizing function only under the condition of neutral or weakly alkaline pH value when it is oversaturated with calcium and phosphorus ions. Therefore, the physical properties of mixed saliva have a significant impact on the homeostasis of solid teeth tissues. In modem literature, work devoted to the study of the mineralizing potential and biophysical characteristics of saliva in children, contradictory. The obtained data on the violation of the mineralizing composition and biophysical properties of the oral fluid during the period of destructing permanent teeth testifies to the appropriateness of the appointment of these ages of both the ex- and endogenous prevention of the caries of the teeth immediately after their rubberization.

  • This article presents reasonable recommendations on the use of intraoperative balloon stretching of tissues to eliminate cicatricial deformities resulting from bums to the face and neck. The obtained morphological results show that the method of intraoperative balloon stretching of tissues ensures the viability of the flap. Output. Thus, intraoperative stretching of soft tissues according to the scheme developed in the clinic does not cause pathological changes in the general architectonics of the skin, does not violate the surface microrelief and the reticular structure of the fiber base of the skin.

  • A typical feature of generalized periodontitis is the rapid destruction of periodontal tissues.   • Abstract Tooth extraction is the most common operation, after which hemorrhagic complications often occur, especially in patients with chronic viral liver disease. This condition is caused by damage to the endothelial lining of blood vessels. Based on this, the purpose of this study was to study the features of endothelial dysfunction before tooth extraction in patients with viral hepatitis. 58 patients with hepatitis В and C with different prescription periods of the disease were examined. In patients with viral hepatitis, an increase in platelet aggregation activity on the effect of an ADP inducer (Tma) by 45% was noted before tooth extraction. The lengthening of the activated recalcification time (AVR) by 37% observed by us in patients with viral hepatitis reflects a deficiency of plasma factors (XII, XI, XIII ) of the blood coagulation system and indicates a state of hypocoagulation. Against this background, high values of alpha-2 macroglobulin in the blood (4 times) and Willebrand factor (15%) and a significant decrease (by 35%) in the content of protein C in the blood of the examined patients were noted. The obtained results of the study indicate that these patients have a narrow band of maintaining hemostatic balance, and the existing balance can easily be transformed into hypo- or hypercoagulation, which requires preventive measures to prevent complications after tooth extraction.

This paper presents the results of optical densitometry of the periapical area according to the data of cone-beam computed tomography of people aged 25 - 45 years, with a low level of DMFT, who do not have diseases of the periapical tissues. Measurement and determination of normal values of optical density was carried out in the periapical area of various groups of intact teeth of the alveolar bone of the upper and lower mandible using the Planmeca apparatus with the Romexis viewer software.

  • On the basis of an analysis of cone beam computed tomography, 50 people (25 men and 25 women) of mature age (from 32 to 59) the study examines the structural features of the dentoalveolar segments in 13, 14, 15, 16, 23, 24, 25 of the upper jaw and 34, 35, 36, 44, 45, 46 of the lower jaw, the retromolar space of the mandible, the frequency of the presence of growths of the mucosa (Schneider membrane) lining the walls of the cavity of the upper jaw, the frequency of perforation of the bottom cavity of the upper jaw by the root tips of the fangs, premolars and first molars, odontometry of 1.3, 1.4, 1.5, 1.6, 2.3, 2.4, 2.5 of the upper jaw and 3.4, 3.5, 3.6, 4.4, 4.5, 4.6 of the lower jaw. The height of the lower jaw bone in men was greater due to the alveolar tissue. The total length of teeth 3.6, 4.6 of the lower jaw, 1.3, 1.4, 1.6, 2.3, 2.4, 2.6 of the upper jaw, the size of the base of the retromolar fossa were observed to be larger in men.The frequency of perforation of the bottom of the upper jaw cavity by the tops of the canine roots and first premolars was higher in men, the size (AP diameter and height) of the maxillary sinuses was larger in men. The study found that the height of the upper jaw bone in men and women did not differ, and the teeth 1.4, 2.4, 1.6, 2.6 of the upper jaw were larger in men, which can be attributed to the coronal section. The obtained data will help improve the procedure of immediate dental implantation and augmentation of the alveolar bone.

  • In the course of the study, publications of the main scientometric databases over the past 10 years were studied. Among the publications in Russian, 35 articles from Elibrary and Cyberleninka and 21 articles from the PubMed database, which are in the public domain, were studied. Among the publications on the surgical treatment of large cysts of the jaws, the most common are cystketomy, cystotomy, and marsupialization (decompression drainage) operations. Cystoectomy operation was used in 68.6% of cases (24 articles) among domestic authors, 42.86% (10 articles) - by foreign authors; cystotomy was used in 22.8% (8 articles) and 23.8% (5 articles) of cases, the method of marsupialization was used in 8.6% of cases (3 articles) by domestic and 28.57% (6 articles) by foreign authors. Cystectomy, despite the increased risk of recurrence, blood clot infection and the development of inflammatory complications, damage to important anatomical structures during surgery, dysfunction of the resected teeth, impaired innervation in the surgical area, the occurrence of pathological fractures and possible patient disability remains the most common operation for treatment patients with extensive jaw cysts. Cystotomy, in comparison with cystectomy, is a more sparing operation, has a lower recurrence rate and the risk of intraoperative and postoperative complications, but the long rehabilitation process docs not allow the use of this treatment method in all clinical situations. The use of the method of decompression drainage (marsupialization) minimizes the likelihood of damage to adjacent anatomical structures, prevents subsequent possible deformity of the jaws, and minimizes the risk of recurrent jaw cysts, the development of postoperative and intraoperative complications in comparison with cystotomy and cystectomy. Despite the advantages over classical methods of treating extensive cysts of the jaws, marsupialization is not widespread among the studied works of domestic and foreign authors.

  • This article reflects the results of the study of 549 patients with fractures of the lower jaw who received inpatient treatment at the Osh Interregional United Clinical Hospital from 2018 to 2020 in the Department of Maxillofacial Surgery. In the first days of admission to the hospital, 307 (55.9%) people had a violation of sensitivity of the skin and oral mucosa. The results of the treatment of 307 patients showed that after 30 days there was already a violation of the sensitivity of the soft tissues of the face and teeth in 96 (31.2%) patients. The remaining 211 (68.8%) patients recovered completely, and after 6 months 96 (31.2%) of the 74 (77.1%) patients examined had a normalization of sensitivity, and 22 (22.9%) continued to have symptoms of trigeminal nen e damage.

  • According to the data of World Health Organisation every year around 17,5 million people die from heart diseases. 85% happens because of heart attack and stroke. 80% of premature heart attacks and strokes could be prevented. Statistically 30% of all patients in dentistry clinic refers to the group of anesthesiological risk patients because of the chronic diseases they have, more often connected with heart illnesses. Most of such patients take some medicine that affect blood coagulation. Drug intake has to be under constant control, especially if patient takes indirect anticoagulants. Nowadays the only opportunity to take the control of blood coagulation is in the specialized clinics, though it is often very hard for patients, and in case of urgent situation practically impossible. In some situations, especially in oral surgery cases, it is necessary to do quick coagulation test. In this work there was done the analysis of most common difficulties in dental clinics doing oral surgery. The aim of this research is to explore the main difficulties of oral surgery help for patients, who takes oral anticoagulants constantly. As materials and methods of the research were used the surveys of dental patients who also has heart diseases in the anamnesis. Conclusion: To improve the quality of dental treatment it is necessary to change the scheme of the routing for the patients with cardiovascular diseases.

The problem is that in places where there is no specialist doctor, maxillofacial surgeon, patients are not diagnosed and appropriate treatment is not obtained, in turn, the acute process takes place in a chronic stage.

To 43 patients after lower jaw resection with disarticulation (on the occasion of neoplasm) the defects werereplaced by vascularized autotransplants of the 2nd radius of pedis and vascularized fibula autotransplants in combination with titanium implants, hi all cases positive results were received. The authors consider that microsurgery' with vascularized fibula autotransplants in combination with titanium implants was an optimal way to replace lower jaw defects after its resection.

  • Surgical treatment of patients with diseases and injuries of the temporomandibular joint (TMJ) is carried out in persons with traumatic injuries and their complications, secondary deforming osteoarthrosis, destructive changes resulting from purulent-inflammatory, tumor and tumor-like processes, congenital and acquired abnormalities, bone ankyloses, as well as destructive processes of bone-cartilage structures with their replacement to transplantation and implantation materials, and is one of the complex, important problems of modern maxillofacial surgery. Today in clinical practice from the traditional materials most widely used: auto-, allo-xenografts, complex autovascularized autografts, as well as various types of implant structures. However, as recent publications show, and clinical practice, the use of these materials for these purposes tends to decrease due to their shortcomings due to resorption or rejection of transplanted materials, death of osteocytes, osteoblasts and destruction of other living cellular structures, including difficulties in using microsurgical technology [1,4,6,8]. Endoprosthesis of the head of the temporomandibular joint, body and branch of the lower jaw is shown in cases where the use of bone grafts of allo- and autogenic origin is impossible.[ 1,4,7,8]. In this direction, studies related to the development of technology for the production of porous and nonporous materials based on titanium nickelide made at the Siberian Institute of Physics and Technology [2] made a huge contribution. Endoprostheses made from this alloy are well tolerated by body tissues, have high biological inertia, lack of toxicity and reply all the requirements for implant materials.

  • A reduction in the number of complications following tooth implantation remains of critical importance to dentistry. All risk factors can be divided into general, including smoking, systemic diseases, patient’s condition following radiotherapy, etc. and local ones comprising poor oral hygiene, periodontal disease, iatrogenic disorders, design and quality of the transgingival implant. In order to better understand inflammatory degenerative processes around dental implants researchers and dental implant manufacturers are forced to improve currently used conservative and surgical methods of treating and preventing these diseases, focusing more on risk factors for the development of peri-implantitis. The development of new surgical and prosthetic techniques, new dental implant systems will contribute greatly to increasing the life span of implant-supported dentures and improving the patient’s quality of life.

  • The functional state of the nasal mucosa in patients was assessed by MTS time using visual (methylene blue) indicators according to S.Z. Piskunov et al. 75 patients with various pathologies and brain injuries in serious condition were examined. The patients were divided into groups: 1st group - patients with severe TBI; 2nd patients with TBI and nasogastric tube (NGT); 3rd patients with severe TBI and nasotracheal intubation (NTI); and the 4th group with TBI and orotracheal intubation (OTI) and the control group, these are patients in the intensive care unit without TBI. We found that when NTI and NGT are in the nasal cavity for more than 4-5 days, there is a risk of nosocomal sinusitis. This alarms us to take preventive measures to prevent sinusitis nosocomia from the first day after hospitalization of the patient.
