  • A large number of publications are devoted to the problems of diagnosis and treatment of herpes viral infection, which makes it possible to consider it sufficiently studied. Thus, despite certain advances in the study of the etiology and pathogenesis of herpes infection in the oral cavity, information in pregnant women with herpetic stomatitis is scarce. In this regard, research on this problem is relevant. The effectiveness of herpes simplex therapy depends both on the effectiveness of the specific antiviral drugs used and on the final immune corrective effect, which, on the one hand, necessitated the study of the immune status of patients suffering from this disease, on the other hand, the development of various schemes of combined treatment using various specific antiviral drugs and immune modulators, and the inclusion ofphysiotherapeutic methods is considered the safest in this period.

  • The aim of this study was to evaluate some aspects of the pathogenesis and diagnosis of teething in newborns based on the study of the dynamics of estrone in breast milk and the saliva of a newborn. 78 practically healthy lactating women with obesity-38 and 30 who were not obese were examined, as wellas their newborns. Object of study, saliva and blood plasma. It was revealed that the hormonal profile of a newborn in saliva with a change in metabolism in lactating women can undergo certain changes. As lactation develops in women with excess div weight, hormone estrone undergo intensive elimination from milk, which leads to a decrease in its content in breastmilk, which can affect metabolic processes in the newborn's dentofacial system.

  • Purpose: to prevent the occurrence and improve the results of treatment of intestinal suture incompetence in peptic ulcer surgery. Analyzed results of 1131 patients with peptic ulcers underwent surgical treatment during since 1996 to 2020 years. Patients are conditionally split into 2 groups (the first group - patients which underwent surgery in the period before 2000 and second group - from 2000 to the present). From all patients at 2% (23 patients) existed insolvency an suther of cults of duodenum or anastomosis. In the first group at relaporotomy was used traditional tactics of the treatment to insolvency sutcher - relaparotomy, resutchering to lead "tampons-cigars" to place of insolvency, drainage of abdominal cavity. In the second group - drainage of cults duodenum through the defect, with nazointestinal drainage, active aspiration ofduodenal contents through drainage and full-fledged return it through nazointestinal drainage and oral nutrition. In the first group lethality was 1,2%, in the second group - 0,2%. Results: when using the proposed technique, among patients with insolvency of the duodenal stump sutures, 2 patients died, which amounted to 0.2% in relation to all operated patients of the second group (out of 719 patients), and in relation to all early postoperative complications -1.1%

  • At present, among the causes of death and disability in Uzbekistan, injuries are in second place, yielding leadership only to cardiovascular diseases. Maxillofacial injuries account for up to 8% of all traumatic human diseases. In modem reconstructive surgery of the maxillofacial area and dental implantology, various osteo-replacement materials are widely used. These materials are used to replace bone defects, reconstruction of the facial skeleton, etc. All bone substitute materials are used to fill the defect in bone tissue through regeneration, but not repair. The efficiency of regeneration directly depends on the type of material used. The review considers the latest literature data on the use of plasmogel in the treatment of injuries and fractures of the mandible. The presented literature review shows the possibilities of using platelet-rich plasmogel in fractures of the lower jaw to stimulate osteogenesis, which is a modem additional tool in the doctor's arsenal that has a positive effect on osteogenesis and additionally stimulates the regenerative capacity of tissues.   • Introduction: In the modem protocol of treatment of patients with partial or complete adentia, implantation surgery is used with the help of implants. One of the most common complications is an incorrectly selected angle of inclination of the implant, which leads to a number of complications in the form of incorrect load distribution during prosthetics and bone resorption. In this regard, the use of 3D templates and virtual planning of dental implantation plays a special role in implantation. Objectives: Justification of the use of navigation templates during planning, as a prevention of complications of implantological surgery. Materials and methods: The study of CBCT images, removal of the spines from the jaws with a cast mass, marking of anatomically important structures, assessing the compatibility of the optimal final position of the teeth, used (Scan Marker 3-Diemme, Cantu Italy) to determine the central occlusion. Top Scan, Open Technology, Rezatto, Brescia, Italy) to obtain images of the jaws and make a navigation template. Results: The use of a surgical template significantly reduces the problem of the surgical result already at the stages of planning the operation.   • Objective: to study the immunological and morphological features of the course of purulent-necrotic processes in the lower extremities against the background of local application of the proteolytic enzyme cucumazim. Material and methods: 352 patients with purulent-necrotic lesions of the lower extremities on the background of DM were examined. Patients were divided into 2 groups depending on the treatment performed. Group I consisted of 112 patients who underwent traditional complex treatment. Group II included 240 patients who underwent complex treatment with local application of the plant-derived proteolytic enzyme cucumazim. The results of treatment were evaluated on the basis of morphological and immunological studies.Results. It was found that purulent-necrotic complications in the lower extremities occur with a pronounced decrease in humoral and cellular immunity, as evidenced by lymphopenia, an imbalance in lymphocyte populations, and inhibition of phagocytic activity of neutrophils and macrophages. Local use of cucumazim, judging by morphological data, allowed the relief of purulent-necrotic processes in the foot on the one hand, on the other, had a significant positive shift in regenerative-proliferative mechanisms in damaged tissues. Against the background of local use of cucumazim, there was not only a tendency to normalize immunity indicators by increasing the quantitative content of cellular elements, but also their qualitative improvement. Conclusion. Based on the conducted studies, it was found that purulent-necrotic complications in the lower extremities occur with a pronounced decrease in humoral and cellular immunity. Local use of cucumazim as part of a complex treatment contributedto the normalization of immunity indicators and the relief of the purulent-necrotic process.

In the first month of life the children has high activity thermogenesis, at the expense of the brown fatty tissue having high power potential. Boys have an activation of heat production of brown fat high, than at girls that provides more high temperature on some body parts. The development and intensification of school education has exacerbated the problem of maintaining the health of students. Among children who begin regular education, the prevalence of borderline health disorders increases with the formation of chronic pathology in the future, therefore, the search for methods of early prenosological diagnosis and prevention of diseases is especially relevant. In this regard, the World Health Organization defines one of the priority areas to support the health, safety and well-being of children and adolescents. Quality standards have been developed for school health services, which should ensure the improvement of the health of young people in all educational institutions. Despite the recognition that the tasks of preserving and strengthening the health of modern schoolchildren can and should be solved directly in an educational institution, universal and effective forms of organizing the educational process have not yet been found, the issue of optimizing motor activity and nutrition of students has not been resolved. Actual development of new conceptual provisions for the organization of prevention using a systematic approach, changing the role of the doctor as the organizer of school health care and evidence-based integration of the teacher into the system for preventing developmental disorders and the health status of school-age children.

  • The article presents the available literature data on combined pathologies with hereditary coagulopathy and includes our own statistical calculations and 4 clinical cases of a combination of Hemophilia A, von Willebrand disease with hard palate defects, the genetic locus of which is located in the immediate vicinity of the F8 gene. Mutations and changes in the protein structure of the F8 gene can lead to the development of both sporadic forms of hemophilia and occur in patients with a hereditary predisposition. Research methods: coagulological, examination and questionnaire data. Conclusion: it is necessary to widely introduce methods of molecular genetic research and prenatal diagnostics in Uzbekistan.

  • The article is devoted to an experimental study of the pharmacokinetics of gentamicin in rats with lymphotropic pretracheal and intramuscular routes of administration. Data on the concentration of the antibiotic in the blood and tissues of the respiratory organs of rats in dynamics are given, it is shown that the method of lymphotropic pretracheal administration provides higher concentrations of the antibiotic in comparison with the intramuscular method. The high therapeutic efficiency of this method can be achieved due to the presence of connections between the lymphatic system, as well as organs and tissues. This circumstance allows you to deliver the drug to the affected organ directly. In addition, in the case of lymphotropic therapy in the focus, the optimal concentration of the drug is maintained for 24 hours. So there is no need for frequent administration of the drug (for example, with antibiotic therapy), you can reduce the course and daily doses of drugs. The drug, which is administered lymphotropically, is not able to have a toxic effect on the intestines, kidneys, liver, since only a tiny part of the drug enters the bloodstream.

  • The development of cytopenic syndrome in chronic liver diseases accompanied by portal hypertension is associated by many researchers with splenomegaly. The article is devoted to the study of the dependence of hypersplenism (cytopenic syndrome) on the magnitude of splenomegaly and the level of portal hypertension, as well as the severity of liver damage in intra- and extrahepatic blockade of portal circulation. The analysis was carried out in 25 children suffering from extrahepatic PH and 46 children with intrahepatic blockade of the portal circulation, who were hospitalized at the age of 3 to 14 years. The development of cytopenic syndrome in children with both intra- and extrahepatic forms of portal hypertension does not depend on the size of splenomegaly. It is reasonable to believe that the cytopenia that develops in children with portal hypertension and splenomegaly is more associated with liver damage and intrahepatic blockade of the portal circulation, as well as the level of portal hypertension.

  • To date, the pathogenesis and morphogenesis of COVID-19 have not been studied in depth, and therefore, in this work, the goal is to study the damage to the parenchymal-stromal elements of the myocardium in COVID-19. The material for the study was pieces from the myocardium of the ventricular wall of 26 patients who died from COVID-19 at the age of 28 to 56 years. The results of a morphological study showed that edema and mucoid swelling of the interstitial tissue of the myocardium in COVID-19 occurs due to the accumulation of hyaluronan, the development of dystrophy, dysregeneration and inflammation of blood vessels and connective tissue structures. In the myocardial parenchyma, contracture disorders, homogeneous compaction of the cytoplasm of cardiomyocytes, disappearance of the transverse striation of myofibrils, focal development of basophilia, PAS-positive metaplasia of cardiomyocytes, fragmentation, degeneration and necrobiosis of muscle fibers were noted; compaction, deformation and destruction of the nuclei of cardiomyocytes. Injuries to cardiomyocytes are first manifested by sarcoplasm vacuolization, a change in the coloration and shape of the nucleus, then enlargement and hyperchromasia of the nucleus in the form of dysplasia occur, homogenization, metachromasia and blue staining are noted from the sarcoplasm, followed by lysis of the nucleus; in the future, metachromasia covers the entire cytoplasm of the cardiomyocyte and the cell dies.

  • The problem of internal picture of disease (IРD) in children with acute leukemia (AL) has not only medical, but also psychological aspects. The psychological study of the personality characteristics of a sick child is one of the ways to reveal the IРD. The purpose of this research work: to study and analyze the clinical and psychological characteristics of IРD and personality in patients with acute leukemia in children of the Uzbek population to optimize psychotherapy, psychocorrection and the processes of their medical and social rehabilitation. Material and Methods: 41 children aged from 7 to 15 years with acute leukemia were examined. The examination program included: experimental, medical and psychological examination of the emotional personal sphere. Results: in patients with АL children of the Uzbek population, the level of personality neuroticism is increased. A complex and dynamic structure of the internal picture of the disease is formed. As a rule, it negatively affects the behavior and mental state of children, interferes with the treatment process and even more maladjusts patients. Such stable personal characteristics as emotional-volitional instability, passivity, self-doubt, lack of sociability, dependence on others, reduced opportunities for social adaptation skills have been identified. Conclusions: The identified personality characteristics significantly reduce the resistance to frustration in a sick child and determine his behavior at the stages of the treatment process. The used psychodiagnostic methods of examination make it possible to determine the psychological characteristics of a sick child, which makes it possible to carry out differentiated psychotherapeutic and psychocorrective work with patients with АP and are of great importance for optimizing the process of their medical and social rehabilitation.

  • To date, the influence of the balance of the content of essential MEs on the ability of children to adapt to the physiological course of this period and the implementation of pathological syndromes has not been sufficiently studied. The purpose of the study is to study the relationship between the deficiency of certain microelements and the risk of violations during the period of early neonatal adaptation in newborns with perinatal posthypoxic encephalopathy. Material and methods of research: To solve the set tasks, 131 "mother-child" pairs in the neonatal period were under our dynamic supervision. To determine the content of microelements in blood serum, we used a photometric-colorimetric method, using kits from the Italian company "Sentinal Diagnastics" and control materials from the company "Randox Laboratorics LTD". In the examined women, depending on the gestational age and parity of pregnancy, the serum concentration of Zn, Fe, Cu and Mg was determined in the III trimester. In children, the dynamics of serum levels of Zn, Fe, Cu and Mg was studied in the early neonatal period: at birth (1st study), day 3 of life (2nd study) and on days 5-7 of life (3rd study). ). Conclusions: The greatest risk of severe complications of neonatal adaptation in the form of AEDs was found in premature infants with low levels of Zn and Mg. Prognostic clinical and laboratory criteria for disorders of ME homeostasis in newborns are: prematurity, the content of erythrocytes and hemoglobin in the blood on the 1st day of life, Apgar score in the first minute, low birth weight, levels of Zn, Fe, Cu and Mg in the blood of mothers , chronic diseases of the gastroduodenal system, the presence of microelementoses in the blood serum of a pregnant woman.

  • The features of regression of congenital hydronephrosis were studied depending on its stage and follow-up period in the long-term period after corrective plastic surgery in the area of the ureteropelvic segment in 81 children, aged from 1 to 14 years, with stages I, II and III of congenital hydronephrosis with one ( 53) and on both sides (28), without postoperative complications. In patients with stage I congenital hydronephrosis 3-6 months after surgery, there was a rapid regression of congenital hydronephrosis. The renal cortical index has completely returned to normal, while in patients with stage II congenital hydronephrosis, the tendency to regression slows down sharply. A decrease in the renal-cortical index to normal values was observed only after 1-2 years. In stage III congenital hydronephrosis, the renal-cortical index in the long-term period decreases by no more than 40%, and after 3 years by 50% compared with the preoperative period. In no case in stage III congenital hydronephrosis did we notice a reduction in the size of the pelvicalyceal system to a level corresponding to a healthy organ, regardless of the timing of the operation. Comparative studies of congenital hydronephrosis before and in the long-term period after surgery showed that the degree and intensity of the reduction in the size of the pelvicalyceal system was the higher, the smaller its initial dimensions were. The data obtained indicate the need for early diagnosis of congenital hydronephrosis and the implementation of surgical corrections in the early stages of the disease.

  • The data of numerous studies on the identification of IDA among the population of Uzbekistan allow us to conclude that this region belongs to a high-risk group. These data require urgent measures to prevent IDA among the population, especially children. The aim of the study was to study the pathogenesis of clinical and laboratory features of iron deficiency in girls aged 12-14 years, permanently residing in the Ferghana Valley of the Republic of Uzbekistan. Object and methods of the study: adolescent girls aged 12-14 years (n=177) of the Uzbek population, respectively, with LAD (45), mild IDA (56) and moderate (25), and the control group (51). For the study, whole blood and serum of adolescent girls with IDA were used. General clinical, instrumental, biochemical and statistical methods for processing the obtained results were used. The data obtained indicate that a decrease in the level of hemoglobin and serum iron in the blood cannot be an objective sign of iron deficiency, their significance increases only with an increase in blood transferrin. Conclusions: a conclusion is made about the priority of iron deficiency in adolescent girls, they have a steady decrease in the number of erythrocytes starting from the LDH stage, where two factors are significant - a decrease in the proliferative activity of nuclear elements, or a shortening of the lifespan of erythrocytes. The decrease in the hemotocrit index (GI) depends both on the overall decrease in the number of erythrocytes per unit volume of blood, and on the increase in the volume of extracellular fluid and plasma.

  • To obtain an adequate qualitative assessment of the activities of the medical staff of the rural area, a survey of doctors and paramedical staff was conducted, data on the age structure, length of service in general and in the specialty, issues of assessing the performance of medical staff and quality control were analyzed. We used sociological research methods - a survey of paramedical personnel was conducted on the basis of the central district hospital of the district. The data of a survey of 60 female paramedical workers were analyzed. The practical significance of the study is to obtain and process data on the performance of the nursing staff of the central district hospital in order to develop recommendations aimed at eliminating a number of factors that negatively affect health and quality of work, which can serve as an obstacle to professional activity.
