The goal of the research was to study the infringements of metabolizmes catecholami- nines and processes of peroxide oxidation of lipids (POL) at the patients with familial hypercholesterinemia. The subjects of the
study were 92 patients with familial hypercholesterinemia and 15 practically healthy individuals. The study condition sympaticadrenal system (SAS) (increase excretions of catecholaminines: adrenaline (A), noradrenaline (NA).dopamine (DA), DOPA), total cholesterol, cholesterol of low - density lipoproteins cholesterol of high - density lipoproteins, triglycerides, processes of peroxide oxidation of lipids and monoamoxydasa in plasma of blood . The study revealed that abnormalities activity of SAS
patients with familial hypercholesterinemia. low activity monoamoxydasa in plasma of blood and significantly higher level of POL, it was verified by increased level of malonic dialdehyde - final substance of peroxidation
Развитие современных перинатальных технологий в настощее время способствует совершенствованию методов выхаживания и оказания специализированной медицинской помощи детям с массой тела менее2500 г и сроком гестации мснее37 недель, что взаимосвязано, в свою очередь, с увеличением доли таких пациентов в структуре новорожденных. [1, 2]. По данным ВОЗ, смертность среди детей с массой менее2500 г, родившихся живми, составляет 14 на 1000. Из них около2/3 умерших приходится на детей с массой тела при рождении менее 1500 гр. [3,4,5]. Среди факторов, повреждающих головной мозг новорожденного, особо выделяют нарушения углеводного обмена. Гипогликемия чрезвычайно опасна в раннем неонатальном периоде для недоношенных детей тем, что, вызывая компенсаторное повышение мозгового кровотока, она может способствовать возникновению и прогрессированию внутрижелудочковых кровоизлияний. В подавляющем большинстве случаев неонатальная гипогликемия протекает бессимптомно.
Atherosclerosis is a chronic lesion of the arteries caused by the growth of multiple dense nodular thickenings of the walls of the artery (plaques), narrowing its lumen and contributing to the formation of a blood clot - a thrombus, which can clog the vessel. Atherosclerosis develops as a result of complex structural changes that occur in the intima (inner layer) and in the media (muscle layer) of the arteries, and it is associated with the accumulation of lipids and mucopolysaccharides in the blood vessels, the growth of connective tissue and the deposition of calcium. Cardiovascular pathology, and this is ischemic heart and brain disease, occlusive diseases of peripheral arteries, is the most common cause of morbidity, mortality and disability in the population of industrialized countries. According to the statistics of the World Health Organization, in the structure of total mortality in Russia, cardiovascular diseases account for 57%. Most of these cases are associated with diseases caused by atherosclerosis [3]. The development of atherosclerotic lesions of the vascular wall is a complex multi-stage process. It has now been established that even before endothelial damage, blood components begin to interact with the endothelial surface. In particular, low-density lipoprotein cholesterol (LDL-C) and their active component apolipoproteins are able to penetrate into the subendothelial space and, being oxidized, affect endothelial cells. In this regard, the initial stage of atherosclerosis is characterized as a response to the retention of atherogenic particles [2]. The main risk factors that play a significant role in endothelial damage are smoking, arterial hypertension and hypercholesterolemia. It has been shown that with an increase in cholesterol levels of more than 8.5 mmol/l, the risk of fatal cardiovascular complications increases by 4 times, when combined with arterial hypertension by 9 times, with smoking, the presence of hypercholesterolemia and arterial hypertension by 16 times [1]. The problem of primary and secondary prevention of ischemic stroke is not only medical, but also of great social importance, since patients who have had it become disabled in more than 80% of cases and often need outside help. Statins are considered highly effective drugs with the greatest evidence in reducing the risk of cardiovascular events in patients with coronary artery disease.
Objects of inquiry: 107 women with rheumatoid arthritis in climacteric period.
Aim of inquiry: studying clinical-immunologic and roentgenologic indices in females with rheumatoid arthritis in connection with climacteric periods and perfecting therapeutic approaches.
Methods of inquiry: clinical studies, assessment of pain syndrom under visual analogous scale (VAS), determination of pro-inflammatory cytokins in blood scrum, roentgendensitometry, determination of body mass index, gravity of climacteric syndrom under modified menopausal index, determination of osteosyndrom by Ritchie and Lee test.
Results achieved and their novelty: for the first time in RA women in climacteric an interrelation between clinical laboratory RA manifestations and an expression of reproductivity disorders has been revealed. An interplay between acute phase indices, rheumatoid arthritis and Lee test and grade of severity of climatcric period has been established. An intcraggravating effect of RA and climacteric syndrom was approved. Predictive valuable factors in RA women in climacteric were determined. Osteoporosis in females with RA was began earlier before climacteric, it lasted more pronounced with lowering mineral density of bones. An advancing lowering of mineral density of bones in RA began in climacteric was shown. Dependance between a level of pro-inflammatory cytokins and duration of disease has been revealed, and less expressed regression of IL-ip indices in women with preserved menstrual function. Recommendations for strategy of RA treatment in women in climacteric taking into consideration features of a course of disease were worked out.
Practical value: rocngcnometry in females with RA, determination of TNF-a, IL-ip cytokins, modified menopausal index by Kuppermann contributed to diagnosis and prediction, management of an adequate therapy. Using of plasmapheresis and climadinon in complex therapy of rheumatoid arthritis reduced manifestations of disease and climacteric syndrom.
An implementation degree and economic efficacy: results of studies were implemented into activities of the Republican Arthrology Center, Specialized Qualified Ambulance Treatment, departments of Rheumatology, Cardiorhcumatology, 1st clinic of the Tashkent Medical Academy, in teaching process on a department of Faculty & Hospital Therapy, Oriental Medicine and Internal Diseases of the medico-preventive faculty.

Патогенетической основой неалкогольной жировой болезни печени (НАЖБП) является феномен инсулинорезистентности, для которого характерно снижение чувствительности тканевых рецепторов к эндогенного инсулина, вырабатываемого в нормальном или повышенном количестве. При этом нарушается поступление глюкозы в клетки, сопровождается повышением скорости липолиза в жировой ткани и увеличением концентрации свободных жирных кислот в сыворотке крови. Гиперинсулинемия способствует снижению скорости -окисление свободных жирных кислот в печени и увеличение синтеза липопротеинов очень низкой плотности. Избыточное поступление свободных жирных кислот в печень с одновременным снижением скорости их окисления, активное образование из них эфиров (триглицеридов) способствуют формированию жировой дистрофии гепатоцитов, которая наиболее выражена при висцеральном ожирении. В условиях стеатоза гепатоцитов развивается эффект липотоксичности.
Хроническая болезнь почек (ХБП) в терминальной стадии приводит к тяжелому метаболическому поражению костей, которое нередко осложняется остеопорозом и переломами. Остеопороз это состояние, характеризующееся низкой костной массой, что приводит к снижению прочности костей и повышенному риску переломов. У пациентов с ХБП это является частью более широкого спектра метаболических проблем с костями.
В исследованиях, охвативших несколько десятилетий, было установлено, что циркулирующие липопротеины и их рецепторы являются основными факторами в атерогенезе. Нобелевские лауреаты Д.Л. Гольдштейн и Г.Р. Браун впервые продемонстрировали, что рецептор липопротеинов низкой плотности (ЛПНП) играет ключевую роль в регуляции гомеостаза холестерина через эндоцитоз частиц ЛПНП. Они также показали, что мутации в гене рецептора ЛПНП приводят к увеличению уровня ЛПНП и холестерина в сыворотке крови, а также повышают риск развития коронарного атеросклероза у пациентов с семейной гиперхолестеринемией [2]. С момента открытия рецептора ЛПНП было идентифицировано множество других родственных рецепторов, которые связываются с нативными или модифицированными липопротеинами [4]. Они влияют на различные физиологические и патологические процессы: эмбриональное развитие, воспаление, гемодинамику, тромбоз, гиперплазию неоинтимы и атеросклероз.
