Поражение легких, вызванное курением электронных сигарет (вейпинг) все чаще встречается у подростков и молодых людей. В англоязычной литературе для обозначение этой патологии используют термин EVALI [1], в русскоязычном переводе‒ повреждение легких, ассоциированное с вейпингом и электронными сигаретами (ПЛАВЭС). Широкое распространение вейпинг получил в нашей стране в последнее десятилетие. Поражение легких токсическими веществами, содержащимися во вдыхаемом аэрозоле, часто не распознается вовремя, что приводит к тяжелым последствиям, а иногда и к гибели подростков. В своевременной диагностике заболевание и выявлении причины его возникновения поможет сбор анамнеза, направленный на выявление факта курение электронных сигарет, клиническая и рентгенологическая симптоматика, морфологические изменения в паренхиме легких. Жалобы, возникающие при поражении легких у вейперов не специфичны. В настоящее время диагноз ПЛАВЭС остается диагнозом исключения. Однако заподозрить токсическое поражение легких можно при появлении лихорадки, снижении массы тела, тахикардии и тахипноэ, кашле, боли в грудной клетке, появлении гемофтиза. Наряду с этим у пациентов могут быть жалобы и со стороны желудочно-кишечного тракта. Кишечная симптоматика зачастую предшествует респираторным симптомам. Дыхательная недостаточность может нарастать в процессе заболевания, примерно треть пациентов нуждается в проведении искусственной вентиляции легких. [1]. Наиболее часто встречающийся паттерн при рентгенологическом исследовании представляет собой двусторонние диффузные инфильтраты в виде «матового стекла», расположенные в базальных отделах и субплеврально. При КТ-исследовании описывают симптомы «дерева в почках» и двусторонние центрилобулярные узелки, имитирующие метастазы. [2].
Интерстициальные заболевания легких (ИЗЛ) у детей раннего возраста часто скрываются под маской пневмонии, бронхиолита, что приводит к поздней диагностике, длительным курсам антибактериальной
терапии и терапии системными стероидами. Для детского ИЗЛ синдрома характерны персистирующее тахипноэ, одышка, кашель, мелкопузырчатые хрипы, гипоксемия, интерстициальные изменения по типу «матового стекла» на КТ легких [1]. Среди детских ИЗЛ особое место занимает заболевание с неустановленной этиологией – нейроэндокринная клеточная гиперплазия младенцев (НЭКГМ). Для этого заболевания характерно сочетание ИЗЛ синдрома и гиперплазии бомбезин-позитивных нейроэндокринных клеток периферических дыхательных путей в биоптате легких [2]. Генетические нарушения метаболизма белков сурфактанта составляют 10% от всех детских ИЗЛ. При наличии мутации генов SFTPC и SFTPА1 респираторных симптомов при рождении может не быть, но позже развивается ИЗЛ, кислородозависимость и необходимость проведения трансплантации легких [3].
Развитие современной торакальной хирургии у детей идет по пути максимально бережного отношения к легочной ткани. До настоящего времени не выработаны единые показания к выбору метода хирургического вмешательства при мальформации легких в зависимости от объема и степени пораженной части легкого. Вторично развивающиеся деформирующие бронхиты и бронхоэктазии, явления функциональной неполноценности в виде прогрессирующей легочно-сердечной недостаточности, которая нарастает по мере увеличения срока, прошедшего после операции, а оставшаяся легочная ткань подвергается эмфизематозным и пневмосклеротическим изменениям после обширных резекций легких, требует дифференцированного подхода при выборе объема и метода оперативного вмешательства при мальформации легких у детей.
In connection with the increase in the incidence of respiratory organs, interest has revived in the study of fundamental issues related to the diversity of lung cells. The aim of the study was to determine the features of the structural organization of the APUD-systcm of human lungs at different stages of embryonic development, as well as in inflammatory and tumor processes in them. The lungs of human fetuses and newborns, as well as children who died from respiratory diseases and in cancer patients were studied by histological methods. After fixation in Bouin's fluid, paraffin sections were stained using general histological methods: hematoxylin and cosin, according to the Van Gicson and Weigert method. To identify argyrophilic neuroendocrine cells, the sections were impregnated with silver nitrate according to the Grimclius method. Neuroendocrine cells (apudocytes) of the lungs arc determined at the tubular and alveolar stages of organ histogenesis. Branching of the bronchial tree is predetermined by closed apudocytes and neuroepithelial bodies. After birth, their number decreases. With maladaptive remodeling of the lungs caused by inflammation and especially by the tumor process, proliferation of apudocytes of a predominantly closed type occurs, and neuroepithelial bodies are also formed.
This article presents a brief review of sepsis/SS, reflecting the unresolved issues of pediatric sepsis and the results of our own study of surgical sepsis in children. Optimization of diagnostics and schemes of pathogenetic intensive therapy of surgical sepsis in children based on clinical and laboratory criteria and
bacteriological monitoring. Research period - 2018-2020. The subject of the study (n=73) was children with surgical pathology (widespread peritonitis, bacterial destruction of the lungs, post-traumatic brain hematomas, trauma to the abdominal organs, etc.). Studies have shown the effectiveness of complex intensive care in 86.3% of cases. Mortality was stated in 13.7% of cases. Patients with severe surgical pathology died: fecal, widespread peritonitis, severe TBI + coma with irreversible neurological disorders, urosepsis against the background of chronic renal failure, after repeated surgical interventions, due to the development of refractory SS.
The paper identifies the clinical, functional and radiological features of COPD in diabetes mellitus, the possibility of predicting its development. The examined patients were divided into 3 groups: patients with COPD in combination with diabetes mellitus, patients with COPD and patients with diabetes mellitus. In the structure of COPD, the pulmonary component and systemic manifestations arc distinguished, which can aggravate the disease. The manifestations of diabetes mellitus are based on micro- and macroangiopathies that affect pulmonary microcirculation. When managing patients with COPD in combination with diabetes mellitus, attention was drawn to more pronounced clinical manifestations of respiratory failure (shortness of breath, participation in breathing of auxiliary muscles, hypoxemia), especially in stage III COPD, compared to patients with COPD. To diagnose microvascular lesions in patients with COPD and diabetes mellitus, to predict the course of nonspecific lung diseases, based on the specific results of the study, multispiral computed tomography was performed. In patients with COPD, the assessment of the degree of impaired ventilation in the lungs should be carried out under CT conditions performed in the inspiratory and expiratory phases. Diagnostically significant for COPD during computed tomography is a symptom of expiratory "air trap", in combination with expansion and deformation of bronchi of various sizes, up to broncho- and bronchioectasis. When MSCT in patients with COPD and diabetes mellitus, pathognomic changes characterizing microangiopathy and fibrotic changes. The clear-cut shape of microvessels is a specific X-ray morphological sign in patients with COPD in combination with diabetes mellitus.
