  • Kidney damage is a common occurrence in patients with rheumatologic diseases and can develop either due to the disease itself or secondary to the drugs used in the treatment. The prevalence of renal damage and its severity depends on the underlying disease, as well as on the aggressiveness of therapy. For most rheumatologic diseases, kidney damage portends a poor prognosis and requires aggressive immunosuppressive treatment. Thus, it is important to diagnose and treat them early. On the other hand, it is almost impossible to group NSBNs according to the greater or lesser adverse effect on the kidney - there are very few comparative scientific studies. But it can be argued that practically all NSPS (including selective COX-2 inhibitors) have side effects on the kidneys. It is important to add that other pain relievers, such as acetaminophen (known to all as paracetamol), in some cases can cause kidney failure.

  • The undoubted and justified interest of researchers in comorbid conditions in rheumatic diseases has not faded away in the last decade, but the concept of multimorbidity has not yet been integrated either into clinical practice or into scientific research in the field of rheumatology. Renal comorbidity is common in patients with rheumatic disease on the basis of regular examination of serum and urine renal function tests. When patients have both arthritis and kidney disease, the following questions should be addressed. Is kidney disease a complication of rheumatic disease or its treatment, or arc they both manifestations of the same systemic autoimmune disease? This article is intended to clarify the above issues and to be used by rheumatologists and nephrologists in the treatment and monitoring of these diseases.

  • Purpose of the study: study and evaluation of biphosphonates in the treatment and prevention of perimenopau-sal osteoporosis. 60 perimenopausal patients were examined. In both groups, age are ranged from 45 to 55. The first group consisted of women who were offered alendronic acid dose of 70 mg per week for 6 months. The second group includes patients who, for economic reasons, did not take the above drug. The examined women determined the level of calcium and alkaline phosphates in the blood. The determination of bone mineral density was carried out using the ultrasonic densitometry method. From the above situation it is obvious that treatment with a group of biphosphonates, in particular alendronic acid 70 mg per week, significantly increases bone mineral density, which in turn prevents atraumatic fractures in women in the perimenopausal period.

  • The effect of complex drug (anti-inflammatory, basic) and non-drug (physiotherapy, procedures of therapeutic gymnastics) therapy on clinical indicators in 80 patients with early osteoarthritis (OA) of the knee joints was studied. It has been established that complex treatment contributes to the effective abating of pain (according to VAS) and the improvement of the motor activity of the knee joint (according to the criteria of experts). At the end of inpatient treatment (10-12 day), the course of OA improves (according to the Lekcn index) and the risk of synovitis is reduced (according to the criteria of L.M. Noskova, 1987).

  • Injuries of the spine and spinal cord account for 0.7-4% of all injuries of the musculoskeletal system [2,6,8]. According to Simo-nova I.A. and Kondakova E.N. (2003) among all of skeletal injuries, about 6.0% are spinal injuries, of which about 30.0-40.0% of injuries are accompanied by neurological deficits. In Russia, annually 600-640 people per 10 million people receive mostly young young and able-bodied age spinal cord injuries, and 8000 people become disabled [11,16]. Over the past 70 years, spinal injuries have increased by more than 200 times, which is associated with urbanization, the development of production and transport [13,16,17].

  • It was examined clinical features, data of joint syndrome, neurotic changes and the level of hormones in blood in 48 patients with rheumatoid arthritis (RA) (women - 32, men - 16) in the age from 60 to 73 years old (in average 69,5±2,2 years) and 40 patients (women - 31, men - 9) in the age from 16 to 59 years old. In patients older 60 years it was often determined the high level of the activity of the disease (according to DAS 28), significant deterioration of roentgenological signs and functional condition of the patients. For this disease it was typical more severe beginning, injures of the big joints such as knee joint, humeral and the ankle and the development of the significant component of exudation due to the asymmetric oligoarthritis. Most accompanying diseases were arterial hypertension (45,8%), ischemic heart disease (41,6%). The carried out research was promoted to reveal the neurotic disorders in 72,7% of patients: in 22,7% - weak, in 35,2% - moderate and in 14,8 % - high degrees of disorders. In group of the patients in the age of till 60 years old neurotic symptoms were revealed in 60,0 % of patients, and in group of the patients older than 60 years - in 83,3% of patients such symptoms were revealed in 1,4 times more often. In group of the patients older 60 years old the level thyrotrophin-releasing hormone (TRH), triiodothyronine, adrenocorticotrophic hormone (ACTH) and cortisol were authentic lower than in patients till 60 years old (P < 0,05 and P < 0,02), and the level thyroxine not always correlated with age and did not correspond to the level of TRH and triiodothyronine.

  • Damage to the knee joint is a serious problem in clinical medicine, as it is a fairly common cause of disability and disability [4, 8, 21]. The knee joint is involved in the pathological process in a variety of diseases - deforming osteoarthritis, rheumatoid arthritis, seronegative spondyloarthritis, microcrystalline arthropathies, chondromatosis, injuries. Injuries of the knee joint occupy one of the first places among all cases of pathology of the musculoskeletal system, and from 43% to 80% of cases are injuries of its ligamentous apparatus [4, 5, 8]. Due to the peculiarities of the anatomical structure of the knee joint, its soft tissue structures are damaged more often than the bone components. Ligamentous apparatus injuries occupy the first place and account for up to 50% of injuries of the knee joint, up to 24% of injuries of the lower limb. Chronic injuries of cartilage, menisci, and cruciate ligaments, which are recorded with a frequency of up to 79%, are the cause of the development of degenerative-dystrophic changes in the knee joint [2]. Degenerative-inflammatory diseases of the joints are a common cause of persistent disability in the population of various age groups. Osteoarthritis is the most common joint disease, occurring in 10-20% of the adult population.

  • Secondary osteoporosis (OP) is a serious complication in patients with rheumatoid arthritis (RA). From 28 to 77% of RA patients have ostcopcnic syndrome and osteoporosis. The most important risk factors for OP in RA include severe disease, the absence of remission in RA, prolonged inadequate therapy with glucocorticoids (GC), and early menopause. There is a direct dependence of the development of OP with a RA of more than 7-10 years. Despite the powerfol anti-inflammatory effect, HA accelerates the development of OP, fractures and patient disability, especially in the first 6 months, taking the drug.
