  • In 49 patients (mean age - 50.6 ± 1.6 years, duration of disease - 9.4 ± 1.7 years) male gout were investigated anthropometric data, lipid profile, depending on the extent of the clinical and laboratory parameters. It has been established that the basic 3 clinical factors of the metabolic syndrome - obesity, hypertension, diabetes mellitus occurs in 49.3% of patients with gout. Dislipedemiya, hypercholesterolemia and increase in low density lipoprotein and very low density detected in 60,0-67,3% cases. In patients with urolithiasis frequency, severity of the violation rate of lower limb function, the number of affected joints, the number of subcutaneous tophi, arthritis incidence in the last year, the duration of the latest exacerbation and severity of gout own index higher than that of patients without metabolic syndrome. The degree of hyperuricemia and duration of the disease have a direct correlation with the degree of increase of total lipids and cholesterol in the blood serum.

  • Experimental study of new polycomposition haemostatic powder was performed. 60 white male rabbits with weight 198 + 2,7 g were used. Morphologic alterations were studied at 3, 12 hours, 1, 3, 7, 14, 30 days after the operation. Results revealed the haemostatic effect of Heprocel powder. Microscopic data showed the gradual healing of the liver wound forming the thin connective tissue in injured zone. The slight inflammation developed as foreign body reaction, which reduced with implant biodegradation. Inflammation signs were manifested in the control group. The complete biodegradation of haemostatic powder Heprocel was registered on 14 – 21 days. Thus, the new haemostatic agent Heprocel might be used for parenchymal bleeding control in trauma and operation.

  • Acute pancreatitis (AP) is one of the most common and severe surgical diseases [27]. In the last decade, there has been a steady increase in the frequency of OP in the structure of surgical diseases of the abdominal organs. At the beginning of the 20th century, OP was described as a casuistry, but at present, in terms of the incidence of diseases of the abdominal organs, it is second only to acute appendicitis and acute cholecystitis [11]. Even 25 years ago, OP was 0.5-1.0% of all surgical diseases of the abdominal organs. At present, its frequency has increased by 10-15 times and, according to various authors, ranges from 4.7% to 15.6% [1]. It should be noted that in developed countries this figure exceeds 10%, while in developing countries this figure ranges from 5-8%. This is due to two reasons: an increase in the absolute number of patients and an improvement in the diagnosis of the disease [12, 31].

  • Epidemiology. Today, more than 230 million people worldwide have diabetes, which is about 6% of the adult population. Diabetes mellitus in Uzbekistan over the past 35 years the number of registered patients increased by 4 times. However, these data do not reflect the prevalence of diabetes in Uzbekistan. Epidemiological surveys were carried out in 3 cities of Uzbekistan showed that the prevalence of diabetes was 10 times higher than in registered patients. Over the past 35 years, 50% of diabetic patients are aged 40-59 years. He has diabetes the prevalence of cardiovascular diseases and stroke among patients by 2-3 times, the risk of vision loss by 10 times, the risk of developing nephropathy by 12-15 times,the development of gangrene of the foot is 20 times higher. Complications of various macro- and microvascular lesions are observed in 50% of patients with type 2 diabetes mellitus. The pre-diabetic state is characterized as a stage of changes in carbohydrate metabolism, leading to clinical manifestations of type 2 diabetes mellitus. This classification is purposefully a disease of the near future included to emphasize the high risk of development. At this stage of a person's carbohydrate metabolism, the disease either stops or diabetes mellitus develops can prevent dangerous complications. Type 2 diabetes develops in 15-30% of individuals who do not make lifestyle and nutritional changes in the pre-diabetic state.   • Patients with diabetes mellitus, often, there are changes in the vessels in the form of calcification of the middle shell of arteries and arterioles (arteriolosclerosis of Menkeberg, mediacalcinosis). The determination of the x-ray stage of calcification of the arteries, gives additional information on the state of the vessels of the foot. Destructive changes in blood vessels, soft tissues and bones, conducted surgical interventions are naturally related. The study of these patterns is possible with a timely, critical study of anamnestic, diagnostic data and therapeutic measures. In this regard, to date remain relevant issues related to the development of differentiated treatment, taking into account all possible diagnostic data and improving surgical methods for treating the diabetic foot syndrome.

  • The main goal of bariatric interventions is to achieve sustained weight loss in obese individuals, which reduces the risk of overall morbidity and mortality. Based on the experience of treatment of 20 obese patients, was established that 6 months after laparoscopic sleeve resection of the stomach (LSRS), which was performed by 16 patients, the body mass index (BMI) decreased from 47,7 ± 2,0 to 38,2 ± 1,4 kg/m2. The installation of intragastric balloon (IGB), carried out by 6 patients with extremely high body weight for the purpose of preoperative preparation before LSRS, helped to reduce the BMI from 71.5 ± 3.0 to 61.3 ± 1.4 kg / m2. The use of IGB and LSRS led to a positive dynamics of the conditions associated with obesity: the number of people with dyslipidemia decreased by 53%; remission ofdiabetes mellitus is achieved in 50%; normalization of blood pressure indicators - in 67%; regression of non-alcoholic fatty liver disease was achieved - in 43% of cases, depression was eliminated in 80%, and mobility disorders - in 65% of cases. IGB and LSRS allowed to improve the Cardiometabolic Disease Staging score, achieving zero cardiometa-bolic risk in 35% of patients, and in the remaining patients - transition to an easier stage.

  • Chronic hepatitis (CG) is characterized by diffuse liver damage, varying degrees of severity of hepatocellular necrosis and inflammation, during which improvement and evolution to cirrhosis of the liver (CP) docs not occur within 6 months.   • The object of the study were 90 white rats at the age of 4-6 months. The morphological changes of the pancreas and the walls of blood vessels of the lower limb in experimental diabetes mellitus. Model of experi-mental diabetes reproduced by single intraperitoneal administration of streptozotocin Wistar rats at a dose of 60 mg / kg. Comparison of the mass of bodies of rat control and experimental group showed that during the experiment, the experimental group of rats had plenty in development compared to the control group by 1.7 times. In all the experiments in terms of pancreatic islets was observed moderate lymphocytic infiltration. Mor-phological examination of the vessels in different time postnatal ontogenesis demonstrated that compared with the control group in the form of changes in the delay of progression and formation of individual components vascular wall.   • The effect of defoliant butyphos (I), dropp (II), butylcaptacs (III), hinazopin (IV), tetra-n-butylammonium bromide (VI), etrel (VII), gemetrel (VIII), allyl-4-methylpyridinium bromide (IX), 1-aminocyclopropan-1-carbonate (ACPC) (X) at various concentrations .(1.10-5-2.10-4 M) on respiration, oxidative phosphorylation (OP) and permeability of the inner mitochondrial membrane from rat liver has been studied. It has been estab-lished that some of the compounds uncouple OP by increasing the inner mitochondrial membrane permeability for H+ (II) inhibit the respiration in V3 condition and induce less selective permeability for a number of ions (I,III). The other defoliants either induce respiration generally in metabolic states 3 and 4 (IV, VI, IX) or have no effect on the respiration and OP (V, VII, VIII, X). On the whole a good correlation between the common tox-icity of the studied preparation (LD 50 ) and their mitochondrial effect has been revealed, therefore the latter can be considered as intracellular targets involved in the realization of pesticide action

  • Materials and methods: 158 male patients with DM type 2 men Uzbek population have been surveyed. Results: Analysis of the research showed that the allele C gene TGFβ1 occurs much less frequently than allele G in both healthy individuals (3,2%), and in patients with type 2 DM of (8,2%). CC genotype frequency of equally high and (93,5%) in healthy patients and DM (84,8; p=0,31%). No significant differences in genotype frequency of CG in healthy subjects and 6,5% of patients with type 2 DM (13,9%). Homozygous GG genotype in healthy men was not present. Conclusion: Except for lack GG genotype among researched healthy men and patients with type 2 DM of Uzbek population no significant difference was defected neither in allele, nor in rep-resentation of genotypes G915C polymorphism gene TGFβ1

  • There are analysed results of surgical treatment of 115 patients with purulent-nekcrotic defeats of bot-tom finitenesses on the background of diabetes mellitus with signs of critical ischemia of bottom finitenesses. The comparative analysis of results of complex treatment has shown, application of Perphtoranum accelerates terms of clarification of wounds from infection for 4-5 days and creates pathological-genetical favorable condi-tions for reduction of quantity of progressing of purulent-necrotic processes of the bottom finitenesses that in its turn leads to reduction of number of the compelled high amputations from 36 % to 12 % and lethality from 15,78 % to 5,17 %, thus average duration of treatment is reduced from 19,5±3,5 days up to 12.5±1.5 days

  • During pregnancy, jaundice syndrome can develop as a complication of pregnancy, as well as in infectious and other pathological conditions. Timely establishment of the correct diagnosis in the development of jaundice in pregnant women is extremely important, since the management of pregnant women with viral hepatitis and women with jaundice due to pregnancy pathologies has significant differences. The development of jaundice in pregnancy can lead to such pathological conditions as indomitable vomiting of pregnant women, cholestatic hepatosis of pregnant women. The most important and difficult is the differential diagnosis of acute fatty hepatosis of pregnant women. Acute fatty liver of pregnancy is a rare occurring pathology of pregnancy: one case of acute fatty hepatosis of pregnant women occurs in 10,000-13,000 births. Consequently, in most maternity hospitals with the number of births of 3000-5000 per year, doctors theoretically have the opportunity to observe such a pathology of pregnant women once every three to four years [1,2].   • The assessment of current of diabetes mcllitus at the 8th child on pompovy insulin therapy by obtaining information from the out-patient card of patients, and also by carrying out poll of patients with diabetes mellitus of children and their parents is carried out. Research has shown that after establishment of insulin pomp the level of glycated hemoglobin in the studied group has decreased that proves efficiency of pompovy insulin therapy. Poll of children and their parents with use of the made questionnaire for patients with diabetes mellitus has revealed improvement of quality of life of patients after establishment of insulin pomp. Children began to feel better, there were more opportunities to be engaged in physical activity, to visit additional circles and more time to find for study.

  • One of frequent complications of diabetes mellitus are trophic and it is purulent - necrotic defeats of feet, as a result of angiopatiya, polyneuropathy, integrated under the name "diabetic foot". The syndrome of diabetic foot meets at 30-80% of patients. At syndrome of "diabetic foot" there is center of infectious defeat, ulcer defect, destructions of deep fabrics of feet which lead to formation of purulent necrotic phlegmon and gangrene of extremities which demand operational treatment. In purulent department of the Samarkand city medical association observation over 67 patients with syndrome of "diabetic foot" is made. Into the present in practice we have entered double-stage operational treatment. At the first stage diabetic phlegmon is opened long rasslablyayushimi cuts with necretomy. At the second stage radical operation is made. Thanks to use of active surgical tactics by us the percent of proximal amputation has decreased from 19% to 4,5%.
