Complex approach to treatment of purulent-necrotic defeats of the bottom finitenesses on the background of diabetes mellitus

There are analysed results of surgical treatment of 115 patients with purulent-nekcrotic defeats of bot-tom finitenesses on the background of diabetes mellitus with signs of critical ischemia of bottom finitenesses. The comparative analysis of results of complex treatment has shown, application of Perphtoranum accelerates terms of clarification of wounds from infection for 4-5 days and creates pathological-genetical favorable condi-tions for reduction of quantity of progressing of purulent-necrotic processes of the bottom finitenesses that in its turn leads to reduction of number of the compelled high amputations from 36 % to 12 % and lethality from 15,78 % to 5,17 %, thus average duration of treatment is reduced from 19,5±3,5 days up to 12.5±1.5 days

Performance evaluation of pulse therapy in patients with systemic lupus erythematosus

The work was carried out examination and treatment of 30 patients with systemic lupus erythematosus (SLE). All patients underwent synchronous intensive therapy (SIT), which included a three-fold plasmapheresis (PF) with removal of 40-60 ml of plasma per kg of patient weight at intervals of 4-5 days. After each PF intra-venously over 45 minutes was administered 1.0 MP, and after the second - more added 1.0 cyclophosphamide. The dose of oral prednisolone should not exceed 40 mg / day. In the second phase was introduced on a monthly basis only 1.0 cyclophosphamide. Complex functional treatment of congenital dislocation of the femur in children

Influence of silicone dioxide nano-particles on prooxidic and proapoptotic effects of lead acetate

The established capability of nanoparticles to enhance the transport of chemicals and drugs into cells and through the barriers of the body makes the possibility of potentiating the toxic effects of chemical contami-nants when introducing into the body together with nanoparticles an urgent matter. In neutrophils of blood by means of cytophluorimetric method, the formation of active forms of oxygen was determined using dichloroflu-orescein diacetate, the intensity of apoptosis and necrosis – by the level of cells labeled with annexin V and propidium iodide, and also the number of cells with reduced mitochondrial transmembrane potential was eval-uated. Maximal changes in all indices were verified in the group of animals administered with SiO 2 nanoparti-cles and lead acetate. In this case, the active forms of oxygen output and the level of apoptotic and necrotic al-tered blood neutrophils of animals and the number of cells with decreased mitochondrial transmembrane po-tential increased significantly in the animals administered with nanoparticles and lead acetate compared to the animals administered with the toxicant only. The conclusion has been drawn that silicone dioxide nanoparticles enhance the capability of lead acetate to increase the production of intracellular active forms of oxygen, the level of apoptotic and necrotic altered blood neutrophils, and the number of cells with decreased mitochondrial transmembrane potential

Peculiarities of the processes of lipoperoxidation and energy exchange in rats of different age groups after the poisoning of their sodium by nitrite on the background of intoxication with tobacco smoke

The intoxication of rats with tobacco smoke triggers the activation of free radical processes, in particu-lar lipoperoxidation, which is confirmed by an increase in the content of TBA-active products in the blood se-rum, liver, lungs and myocardium of rats after injury. The use of sodium nitrite as an additional toxicant deep-ens the activity of lipid peroxidation processes, to which older animals are more sensitive. It was noted that in the conditions of poisoning of rats with both toxicants, the activity of succinate dehydrogenase and cytochrome oxidase in the liver, lungs and myocardium is suppressed, which indicates a decrease in the activity of energy supply processes. The most pronounced decrease in the activity of these enzymes was observed in immature animals 72 hours after sodium nitrite entered the body against a background of 30-day intoxication with tobac-co smoke.

Concerning a fracture of a neck of a femur 65 patients in particular have been operated total endopros-thesis replacement of a joint is carried out. Under our observation there were 65 patients who have executed primary endoprosthesis replacement of a coxofemoral joint concerning femur neck fractures. Among treated 28 - 60 years (43%) are more senior. The most elderly patient was 84 years old. – 22 (33.8%), 80 years – 2 pa-tients – 3% are also more senior than patients from 40 to 50 years. Feature of the early postoperative period was the fact that the patients operated after 3 days from the moment of receipt, much more difficult also are longer the patients operated within the first 3 days adapted to walking on crutches and to service of, than. Good results are received at 86% of the operated patients. Treatment of children with fractures of bones of a forearm is an urgent and complex problem. The analy-sis of treatment of the closed fractures of bones of a forearm at 118 patients is made. The patient has applied conservative treatment (a one-stage manual repozition and an immobilization with plaster steak) and expedi-tious treatment (an open repozition, an intramedullary osteosynthesis and an immobilization with plaster steak) methods. Renewal of working capacity has happened in 71,4% of cases. It has become possible thanks to a combination of terms of treatment to the rehabilitation period, and also use improved a method of expeditious treatment. The correct comparison of bone fracture, their reliable fixing, early adequate functional loading is the main principles of treatment of fractures of forearm. Studying results of clinical application of a way a one-stage manual repozition and an intramedullary osteosynthesis of fractures of bones of a forearm at children is given in article. Results: Analysis of the research showed that the allele C gene TGFβ1 occurs much less frequently than allele G in both healthy individuals (3,2%), and in patients with type 2 DM of (8,2%). CC genotype frequency of equally high and (93,5%) in healthy patients and DM (84,8; p=0,31%). No significant differences in genotype frequency of CG in healthy subjects and 6,5% of patients with type 2 DM (13,9%). Homozygous GG genotype in healthy men was not present. The comparative quantitative assessment risk factors of rapid development of alcoholism are scrutinized in the article. 140 alcoholic patients aged 25-65 years have been studied by Clinical-statistic method. Univari-ate depression analysis with the usage of Kraskola-Uolis criteria was used for comparison with medieval clinic significance, age-specific and dynamical parameters. While carrying out the research the author has revealed that early formation of alcoholic dependence is not associated with the age onset of alcohol consumption but mainly risk factors in premorbid. The results of the research showed that the dynamics alcoholism development is important to evaluate the clinical severity, the flow rate and development of optimal tactics of treatment. The presence of premorbid cofactors risk increases the development of alcoholism as quickly as 2,5 times compared to uncomplicated premorbid. In conclusion, the author draws a conclusion that quantitive dynamic develop-ment characteristics of the disease will be fairly objective criteria to predict the clinical versions and types of alcohol flow.

Clinical checkup, digital investigation of the rectum and rectoromanoscopy, transabdominal and transrectal ultrasound, CT and bacteriological investigation of wound’s discharges were done in all the patients. Course features of hemispheric tumors of the cerebellum

The study includes data from clinical and neurological examinations of 31 patients (18 women, 13 men), whose average age was 30 years with tumors localized in 18 (58,0%) patients in left, 13 (42,0%) patients in the right hemisphere of the cerebellum. For tumors of the hemispheres of the cerebellum characteristic was the presence of general cerebral and focal symptoms. Among the cerebral symptoms were observed headache (77,4%), vomiting (67,7%) and dizziness (35,4 %), of focal disorders - dysfunction of the cranial nerves (80,0%), dominated oculomotory disorders (37,2%), indicating the progression of the compression of brain-stem.
